Description
Overview of the medicine
Imiquimod 5% cream is a topical immune response modifier used to treat certain skin conditions, including external genital and perianal warts, actinic keratosis, and superficial basal cell carcinoma.
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected area, usually 3-5 times a week, for several weeks or months depending on the condition. Specific dosing varies by indication.
Elderly
No specific dosage adjustment required, but caution with sensitive skin.
Renal_impairment
No dosage adjustment needed as systemic absorption is minimal.
How to Take
For topical use only. Apply a thin layer to the clean, dry affected area at bedtime. Do not cover with occlusive dressings. Wash hands before and after application. Wash off with mild soap and water after the recommended application period.
Mechanism of Action
Imiquimod acts as a Toll-like receptor 7 (TLR7) agonist, leading to the induction of local cytokine production, including interferon-alpha, TNF-alpha, and IL-6. This stimulates the innate and adaptive immune responses against the affected cells.
Pharmacokinetics
Onset
Effects observed after several weeks of treatment.
Excretion
Excreted in urine and feces.
Half life
Not well established for topical use, but systemically absorbed drug has a short half-life (approx. 2-10 hours).
Absorption
Minimal systemic absorption after topical application.
Metabolism
Metabolized by the liver if systemically absorbed, primarily to inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to imiquimod or any component of the cream
- •Use on open wounds or broken skin
Drug Interactions
Immunosuppressants
May reduce the effectiveness of imiquimod.
Corticosteroids (topical)
Concurrent use may alter local immune response.
Storage
Store below 25°C. Do not freeze. Keep out of reach of children.
Overdose
Topical overdose is unlikely to cause systemic toxicity due to minimal absorption. Local skin reactions (erythema, erosion, scabbing) may be more severe. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Use in pregnancy only if clearly needed and the potential benefits outweigh the potential risks to the fetus. It is unknown whether imiquimod is excreted in human milk; caution should be exercised when administered to a nursing woman.
