Imotil Plus 2 mg Tablet is a combination medicine containing Loperamide Hydrochloride and Simethicone. It is used to treat symptoms of acute diarrhea including cramps, bloating, and gas. Loperamide reduces the frequency of bowel movements, while Simethicone helps to relieve gas and associated abdominal discomfort.
Uses & Indications
Dosage
Adults
Initially 2 tablets (4 mg Loperamide), followed by 1 tablet (2 mg Loperamide) after each loose stool. Do not exceed 4 tablets (8 mg Loperamide) in 24 hours.
Elderly
No specific dosage adjustment required, but use with caution.
Renal_impairment
No dosage adjustment required.
Hepatic_impairment
Use with caution due to extensive first-pass metabolism of Loperamide. May require dosage reduction.
How to Take
Orally, swallowed whole with water, with or without food.
Mechanism of Action
Loperamide binds to opiate receptors in the gut wall, reducing intestinal motility and fluid/electrolyte secretion. Simethicone is an antiflatulent that reduces the surface tension of gas bubbles in the gastrointestinal tract, causing them to coalesce and be more easily expelled.

Onset
Loperamide: 1-2 hours. Simethicone: Rapid, within minutes.
Excretion
Loperamide and its metabolites are primarily excreted in the feces; negligible renal excretion. Simethicone is excreted unchanged in feces.
Half life
Loperamide: Approximately 9-14 hours.
Absorption
Loperamide is poorly absorbed from the GI tract (approximately 0.3%). Simethicone is not absorbed systemically.
Metabolism
Loperamide is extensively metabolized by the liver (oxidative N-demethylation) via CYP3A4 and CYP2C8.
Side Effects
Contraindications
- •Hypersensitivity to Loperamide, Simethicone, or any component.
- •Patients with bloody stools or high fever.
- •Patients with bacterial enterocolitis (e.g., Salmonella, Shigella, E. coli O157:H7).
- •Patients with pseudomembranous colitis associated with broad-spectrum antibiotic use.
- •Children under 6 years of age.
Drug Interactions
Desmopressin
Oral desmopressin plasma concentrations may be increased by Loperamide.
Other antidiarrheals
Concurrent use not recommended without medical advice.
Quinidine, Ritonavir, Itraconazole, Gemfibrozil
May increase Loperamide levels due to inhibition of CYP3A4 or P-glycoprotein.
Storage
Store in a cool, dry place below 30°C, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of Loperamide overdose include central nervous system depression (stupor, coordination impairment, somnolence, miosis, muscle hypertonia, respiratory depression), urinary retention, and ileus. Cardiac events (QT prolongation, Torsades de Pointes) can also occur. Simethicone overdose is unlikely to cause adverse effects due to lack of systemic absorption. Treatment for Loperamide overdose is symptomatic and supportive; naloxone may be used to reverse opioid effects, with monitoring for recurrence.
Pregnancy & Lactation
Use during pregnancy only if clearly needed and the potential benefits outweigh the potential risks to the fetus. Small amounts of loperamide may appear in breast milk; caution should be exercised when administered to a nursing mother. Consult a doctor.
