Description
Overview of the medicine
Inco SR (Indapamide) is a thiazide-like diuretic primarily used to treat essential hypertension (high blood pressure) and edema associated with congestive heart failure. It helps the body get rid of excess sodium and water, thereby lowering blood pressure.
Uses & Indications
Dosage
Adults
1.5 mg once daily, preferably in the morning. The maximum dose is 1.5 mg daily.
Elderly
No specific dosage adjustment is generally required for elderly patients with normal renal function, but renal function should be monitored regularly.
Renal_impairment
Contraindicated in severe renal impairment (creatinine clearance < 30 mL/min). In moderate renal impairment, careful monitoring is required.
How to Take
Take Inco SR tablets orally once daily, preferably in the morning, with or without food. Swallow the tablet whole with water; do not chew, crush, or break it.
Mechanism of Action
Indapamide acts on the cortical diluting segment of the nephron to inhibit the reabsorption of sodium, leading to increased excretion of sodium and water. This diuretic effect, along with its direct vasodilatory actions, contributes to its blood pressure lowering effect.
Pharmacokinetics
Onset
Onset of action is typically within 1 to 2 hours.
Excretion
Approximately 70% of the dose is excreted via urine and 20-30% via feces, mainly as metabolites.
Half life
The elimination half-life is approximately 14 to 23 hours for the sustained-release formulation.
Absorption
Rapidly and completely absorbed from the gastrointestinal tract. Food does not significantly affect absorption.
Metabolism
Extensively metabolized in the liver.
Side Effects
Contraindications
- •Severe renal insufficiency.
- •Hepatic encephalopathy or severe hepatic impairment.
- •Hypokalemia.
- •Hypersensitivity to indapamide or other sulfonamide derivatives.
Drug Interactions
NSAIDs
Non-steroidal anti-inflammatory drugs (NSAIDs) may reduce the antihypertensive effect of Indapamide and increase the risk of acute renal failure.
Lithium
Concurrent use with lithium may increase plasma lithium levels and lead to toxicity. Monitor lithium levels closely.
Corticosteroids
Concomitant use with corticosteroids may enhance the hypokalemic effect of Indapamide.
Digitalis glycosides
Hypokalemia induced by Indapamide can increase the risk of digitalis toxicity.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include nausea, vomiting, weakness, electrolyte disturbances (e.g., hypokalemia, hyponatremia), and hypotension. Treatment is symptomatic and supportive, including correction of fluid and electrolyte imbalances.
Pregnancy & Lactation
Indapamide is not recommended during pregnancy unless clearly necessary, as there is insufficient data on human pregnancy. It is excreted in breast milk, so it is not recommended during lactation.
