Description
Overview of the medicine
Indaren-G 110 mcg Inhalation Capsule is a combination medicine containing indacaterol (a long-acting beta-2 adrenergic agonist) and glycopyrronium (a long-acting muscarinic antagonist). It is used to relieve symptoms of chronic obstructive pulmonary disease (COPD) by relaxing the muscles of the airways and keeping them open, making breathing easier.
Uses & Indications
Dosage
Adults
One capsule (Indacaterol 110 mcg / Glycopyrronium 50 mcg) inhaled once daily using the inhalation device.
Elderly
No dose adjustment required.
Renal_impairment
No dose adjustment required for mild to moderate impairment. Use with caution in severe impairment or end-stage renal disease requiring dialysis.
How to Take
For oral inhalation only. Do not swallow the capsule. Administer using the provided inhalation device, piercing the capsule and inhaling the powder.
Mechanism of Action
Indacaterol is a LABA that stimulates beta-2 adrenergic receptors in the lungs, leading to relaxation of bronchial smooth muscle and bronchodilation. Glycopyrronium is a LAMA that blocks muscarinic M3 receptors in the airways, preventing acetylcholine from causing bronchoconstriction and mucus secretion. The combination provides sustained bronchodilation.
Pharmacokinetics
Onset
Within 5 minutes for bronchodilation.
Excretion
Indacaterol: Feces (primarily), urine. Glycopyrronium: Urine (majority of systemic exposure), feces.
Half life
Indacaterol: ~40-56 hours. Glycopyrronium: ~15 hours.
Absorption
Indacaterol: Rapidly absorbed, Cmax at 15-30 min. Glycopyrronium: Low systemic exposure, Cmax at 5 min.
Metabolism
Indacaterol: Primarily via UGT1A1, to a lesser extent CYP3A4. Glycopyrronium: Minimal metabolism, mainly by ester hydrolysis.
Side Effects
Contraindications
- •Hypersensitivity to indacaterol, glycopyrronium, or any excipients.
- •Acute bronchospasm (not for rescue therapy).
Drug Interactions
Diuretics
May increase hypokalemia risk with high doses of LABAs.
Other LABAs
Avoid concomitant use as it may increase side effects.
Beta-blockers
May reduce the effect of indacaterol, use with caution.
Anticholinergics
Avoid concomitant use with other anticholinergic drugs as it may potentiate anticholinergic side effects.
MAOIs, tricyclic antidepressants
May potentiate indacaterol's effect on the cardiovascular system.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include tachycardia, tremors, palpitations, headache, urinary retention, blurred vision, and dry mouth. Treatment is supportive and symptomatic.
