Overview of the medicine
Intas 100 mg Tablet contains Sertraline, an antidepressant used to treat depression, obsessive-compulsive disorder (OCD), panic disorder, post-traumatic stress disorder (PTSD), and social anxiety disorder.
Uses & Indications
Dosage
Adults
For most indications, initial dose 25-50 mg once daily, gradually increased to 50-200 mg once daily based on response. Max 200 mg/day.
Elderly
Lower initial doses (e.g., 25 mg/day) may be considered, with careful titration due to potential for increased sensitivity.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. Use with caution in severe impairment.
How to Take
Take orally once daily, with or without food. To minimize gastrointestinal upset, it may be taken with food.
Mechanism of Action
Sertraline selectively inhibits the reuptake of serotonin (5-HT) by presynaptic neurons, thereby increasing the concentration of serotonin in the synaptic cleft and enhancing serotonergic neurotransmission.
Pharmacokinetics
Onset
Antidepressant effects may take 2-4 weeks to become apparent, though some patients may feel improvement earlier.
Excretion
Excreted in both urine and feces, with about 40-45% in urine and 40-45% in feces as metabolites.
Half life
Approximately 26 hours for sertraline, with an active metabolite (desmethylsertraline) having a longer half-life (62-104 hours).
Absorption
Well absorbed from the gastrointestinal tract; peak plasma concentrations achieved in 4.5 to 8.4 hours.
Metabolism
Extensively metabolized in the liver, primarily via N-demethylation, mainly by CYP2C19, CYP2B6, CYP2D6, CYP3A4, and CYP2C9.
Side Effects
Contraindications
- •Concomitant use with MAOIs (risk of serotonin syndrome)
- •Hypersensitivity to sertraline or excipients
- •Concomitant use with pimozide
- •Concomitant use with disulfiram (for oral solution due to alcohol content)
Drug Interactions
MAOIs
Risk of serious, sometimes fatal, serotonin syndrome.
Pimozide
Increased pimozide levels and cardiotoxicity.
Triptans
Increased risk of serotonin syndrome.
Warfarin
Potentiation of anticoagulant effect.
NSAIDs/Aspirin
Increased risk of bleeding.
Other SSRIs/SNRIs
Increased risk of serotonin syndrome.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include somnolence, vomiting, tachycardia, tremor, agitation, dizziness. Management involves supportive care, maintaining airway, monitoring cardiac and vital signs. Activated charcoal may be considered.
Pregnancy & Lactation
Category C for pregnancy. Use only if potential benefit justifies potential risk to the fetus. Small amounts excreted in breast milk; caution advised. Consult a doctor.
