Description
Overview of the medicine
Inteum 50 mg tablet contains Etoricoxib, a selective COX-2 inhibitor, used for symptomatic relief of pain and inflammation in conditions like osteoarthritis, rheumatoid arthritis, and acute gout.
Uses & Indications
Dosage
Adults
Osteoarthritis: 30 mg or 60 mg once daily. Rheumatoid Arthritis/Ankylosing Spondylitis: 60 mg or 90 mg once daily. Acute Gout: 120 mg once daily (maximum 8 days). Acute Pain/Primary Dysmenorrhoea: 120 mg once daily (maximum 8 days).
Elderly
No specific dose adjustment is generally needed, but caution should be exercised. Start with the lowest effective dose.
Renal_impairment
Severe renal impairment (creatinine clearance <30 mL/min) is contraindicated. For moderate impairment (creatinine clearance 30-60 mL/min), a maximum daily dose of 60 mg is recommended.
How to Take
Take orally, with or without food. Administer once daily. Do not exceed the recommended dose.
Mechanism of Action
Etoricoxib selectively inhibits the cyclooxygenase-2 (COX-2) enzyme, which is responsible for the synthesis of prostaglandins, mediators of pain and inflammation. By inhibiting COX-2, it reduces prostaglandin production, thereby alleviating pain and inflammation.
Pharmacokinetics
Onset
Onset of action is approximately 24 minutes for acute pain conditions.
Excretion
Excreted predominantly by the kidneys (approximately 70% of the dose) and to a lesser extent in feces (approximately 20%).
Half life
The mean elimination half-life is approximately 22 hours.
Absorption
Well absorbed after oral administration, with an average oral bioavailability of approximately 100%. Peak plasma concentrations are reached in about 1 hour.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 (CYP) enzymes, mainly CYP3A4, to inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to Etoricoxib or any component of the tablet
- •Active peptic ulceration or gastrointestinal (GI) bleeding
- •Severe hepatic dysfunction
- •Severe renal dysfunction (creatinine clearance <30 mL/min)
- •Inflammatory bowel disease
- •Congestive heart failure (NYHA II-IV)
- •Established ischemic heart disease, peripheral arterial disease, and/or cerebrovascular disease
Drug Interactions
Lithium
Increases lithium plasma levels, potentially leading to toxicity.
Warfarin
Increased risk of bleeding.
Diuretics
May reduce the natriuretic effect of loop and thiazide diuretics.
Rifampicin
May decrease Etoricoxib plasma concentrations.
Methotrexate
Increases methotrexate plasma concentrations, enhancing toxicity.
ACE Inhibitors/ARBs
May reduce the antihypertensive effect and worsen renal function.
Oral Contraceptives
May increase plasma concentrations of oral contraceptives.
Storage
Store below 30°C in a dry place. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, supportive and symptomatic treatment should be initiated. There is no specific antidote for Etoricoxib. Hemodialysis is not effective for removal of Etoricoxib.
Pregnancy & Lactation
Not recommended during pregnancy, especially in the third trimester, due to potential adverse effects on the fetus (e.g., premature closure of the ductus arteriosus). Avoid use during lactation as Etoricoxib is excreted in breast milk.
