Description
Overview of the medicine
Ipralin 100 mcg Inhaler contains Salbutamol, a short-acting beta-2 agonist used to relieve symptoms of asthma and chronic obstructive pulmonary disease (COPD) such as wheezing, shortness of breath, and chest tightness. It also prevents exercise-induced bronchospasm.
Uses & Indications
Dosage
Adults
For relief of acute symptoms: 1 or 2 puffs (100 or 200 mcg) as needed. To prevent exercise-induced bronchospasm: 2 puffs (200 mcg) 10-15 minutes before exercise. Do not exceed 8 puffs (800 mcg) in 24 hours.
Elderly
The dosage is generally similar to adults, but cautious use is advised due to potential increased sensitivity to beta-adrenergic agonists.
Renal_impairment
No specific dosage adjustment is usually required for inhaled salbutamol in patients with renal impairment, but caution is advised in severe cases.
How to Take
Ipralin 100 mcg Inhaler is for oral inhalation only. Shake the inhaler well before each use. Prime the inhaler (release a few test sprays into the air) if it's new or hasn't been used for a week or more. Follow your doctor's instructions for proper inhalation technique.
Mechanism of Action
Salbutamol selectively stimulates beta-2 adrenergic receptors primarily located in the bronchial smooth muscle. This stimulation leads to the activation of adenyl cyclase, which increases intracellular cyclic AMP (cAMP) levels. Elevated cAMP relaxes bronchial smooth muscle, resulting in bronchodilation and improved airflow.

Onset
Bronchodilation typically occurs within 5-15 minutes after inhalation.
Excretion
Excreted mainly in the urine, both as unchanged drug and its metabolites. Faecal excretion accounts for a smaller proportion.
Half life
The plasma half-life of inhaled salbutamol is approximately 4-6 hours.
Absorption
Rapidly absorbed from the lungs following inhalation, with a small portion absorbed from the gastrointestinal tract after swallowing. Peak plasma concentrations are reached within 2-3 hours.
Metabolism
Primarily metabolized in the liver to an inactive sulfate conjugate (salbutamol 4'-O-sulfate).
Side Effects
Contraindications
- •Hypersensitivity to salbutamol or any of the excipients.
- •Should not be used to arrest uncomplicated premature labor or threatened abortion.
Drug Interactions
Diuretics
May lead to hypokalemia, especially with high doses of salbutamol.
Other sympathomimetic agents
Concurrent use may lead to additive cardiovascular effects.
Beta-blockers (non-selective)
Can cause severe bronchospasm in asthmatic patients and antagonize the bronchodilator effect of salbutamol.
MAO Inhibitors (MAOIs) and Tricyclic Antidepressants (TCAs)
May potentiate the cardiovascular effects of salbutamol.
Storage
Store below 30°C. Protect from direct sunlight and freezing. Do not puncture, burn, or expose the canister to heat, even when empty. Keep out of reach of children.
Overdose
Symptoms of overdose include tachycardia, tremors, headache, restlessness, and hypokalemia. Management involves supportive and symptomatic treatment, including monitoring potassium levels and cardiac function. Beta-adrenergic blocking agents may be considered but should be used with extreme caution in patients with a history of bronchospasm.
Pregnancy & Lactation
Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. Salbutamol is likely excreted in breast milk, so use with caution in breastfeeding mothers and only if clinically necessary.
