Description
Overview of the medicine
Irigon-15-Solution is an iron sucrose complex used for intravenous iron replacement in patients with iron deficiency anemia who cannot tolerate or respond to oral iron therapy, or in patients with chronic kidney disease (CKD) on dialysis receiving erythropoietin.
Uses & Indications
Dosage
Adults
Usually 100 mg elemental iron administered slowly intravenously over 2-5 minutes or as an infusion over 15-30 minutes, 1-3 times per week. Cumulative dose can be up to 1000 mg over several sessions.
Elderly
No specific dosage adjustment for elderly patients is generally required, but caution is advised.
Renal_impairment
Dosage as per adult recommendations for CKD patients, often managed by healthcare professionals based on iron status.
How to Take
Administer intravenously as a slow injection or diluted in 0.9% NaCl solution for intravenous infusion. Do not administer by intramuscular injection. Must be administered by a healthcare professional.
Mechanism of Action
Irigon-15-Solution delivers elemental iron directly to the reticuloendothelial system, where it is released and incorporated into transferrin. Transferrin transports iron to the bone marrow for hemoglobin synthesis and also replenishes iron stores in the body.
Pharmacokinetics
Onset
Rapid onset of action, with iron becoming available for erythropoiesis within hours.
Excretion
Sucrose is excreted renally. Iron is primarily stored or recycled; minimal renal excretion of intact iron sucrose complex occurs.
Half life
The elimination half-life of iron sucrose complex is approximately 6-20 hours. Iron is cleared from plasma within 24 hours.
Absorption
Directly absorbed into the systemic circulation upon intravenous administration.
Metabolism
The iron sucrose complex dissociates into iron and sucrose, which are then metabolized separately. Iron is largely taken up by cells of the reticuloendothelial system.
Side Effects
Contraindications
- •Known hypersensitivity to iron sucrose or any of its components.
- •Anemia not caused by iron deficiency (e.g., hemolytic anemia).
- •Iron overload (e.g., hemochromatosis, hemosiderosis).
- •First trimester of pregnancy.
Drug Interactions
ACE Inhibitors
May increase the risk of hypersensitivity reactions in some patients (uncommon).
Oral Iron Preparations
May reduce the absorption of oral iron; oral iron should be started only after the course of IV iron is completed.
Storage
Store below 30°C. Do not freeze. Protect from light. Keep out of reach of children.
Overdose
Overdose can lead to iron overload. Symptoms include hypotension, collapse, nausea, vomiting, and abdominal pain. Treatment involves supportive measures and iron chelating agents (e.g., deferoxamine) if necessary.
Pregnancy & Lactation
Pregnancy Category B. Use in pregnancy only if clearly needed and the potential benefit outweighs the potential risk to the fetus. It is not known whether iron sucrose is excreted in human milk. Exercise caution when administering to a nursing woman.
