Overview of the medicine
Irmukin 80 mg Tablet contains Irbesartan, an angiotensin II receptor blocker, used to treat high blood pressure (hypertension) and to protect the kidneys in patients with type 2 diabetes and hypertension.
Uses & Indications
Dosage
Adults
Initial dose 150 mg once daily, may be increased to 300 mg once daily. For diabetic nephropathy, usual maintenance dose is 300 mg once daily.
Elderly
No initial dosage adjustment typically required, but monitor renal function.
Renal_impairment
No initial dosage adjustment needed for mild to moderate renal impairment. For severe renal impairment or dialysis patients, lower initial dose (e.g., 75 mg) may be considered, with careful monitoring.
How to Take
Taken orally, with or without food, usually once daily.
Mechanism of Action
Irbesartan selectively blocks the binding of angiotensin II to the AT1 receptor, preventing vasoconstriction and aldosterone secretion, leading to vasodilation and reduced blood pressure.
Pharmacokinetics
Onset
Peak plasma concentrations reached 1.5-2 hours. Onset of effect within 1-2 hours, maximum effect within 4-6 weeks of therapy.
Excretion
Approximately 20% excreted in urine (less than 2% as unchanged drug) and 80% in feces.
Half life
Terminal elimination half-life is 11-15 hours.
Absorption
Rapidly and well absorbed from the gastrointestinal tract; bioavailability is 60-80%.
Metabolism
Minimally metabolized by hepatic CYP2C9; main metabolite is the glucuronide conjugate.
Side Effects
Contraindications
- Hypersensitivity to Irbesartan or any component of the formulation
- Pregnancy (second and third trimesters)
- Concomitant use with aliskiren in patients with diabetes or renal impairment
Drug Interactions
Lithium
Increased serum lithium concentrations and toxicity.
Aliskiren
Concomitant use contraindicated in patients with diabetes or renal impairment due to increased risk of renal impairment, hyperkalemia, and hypotension.
NSAIDs (e.g., Ibuprofen)
Reduced antihypertensive effect and increased risk of renal impairment.
Diuretics (e.g., Furosemide)
Increased risk of hypotension.
Potassium-sparing diuretics (e.g., Spironolactone) / Potassium supplements
Increased risk of hyperkalemia.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms: Hypotension and tachycardia, bradycardia could occur. Management: Symptomatic and supportive treatment. If recent ingestion, consider gastric lavage. Hemodialysis is not effective in removing Irbesartan.
Pregnancy & Lactation
Pregnancy: Category D (second and third trimesters) due to fetal toxicity; discontinue if pregnancy is detected. Lactation: Not recommended, as it is unknown if Irbesartan is excreted in human milk, and potential harm to the infant.
