Description
Overview of the medicine
Itopride is a prokinetic agent used to treat gastrointestinal symptoms such as functional dyspepsia, heartburn, nausea, and vomiting by enhancing gastric motility.
Uses & Indications
Dosage
Adults
50 mg three times daily before meals. Dosage may be adjusted based on symptom severity and patient response.
Elderly
No specific dosage adjustment is usually needed, but cautious use is advised due to potential age-related physiological changes. Initiate with the lowest effective dose.
Renal_impairment
Use with caution. Dosage reduction might be considered in patients with severe renal impairment (creatinine clearance <30 mL/min).
How to Take
Take orally, preferably before meals, with a glass of water. Do not crush or chew the tablet.
Mechanism of Action
Itopride increases acetylcholine concentration by inhibiting dopamine D2 receptors and acetylcholinesterase, thereby enhancing gastrointestinal motility and accelerating gastric emptying.
Pharmacokinetics
Onset
Therapeutic effects usually manifest within 30 minutes to 1 hour after administration.
Excretion
Mainly excreted via urine as metabolites, with a small amount excreted in feces.
Half life
The elimination half-life is approximately 6 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract. Peak plasma concentration (Cmax) is reached within 30-45 minutes.
Metabolism
Primarily metabolized in the liver by flavin-containing monooxygenase (FMO3) to inactive metabolites.
Side Effects
Contraindications
- •Known hypersensitivity to itopride or any component of the formulation.
- •Patients with gastrointestinal hemorrhage, mechanical obstruction, or perforation where increased GI motility could be harmful.
Drug Interactions
Warfarin
No clinically significant interaction reported, but monitor INR for patients on warfarin.
Anticholinergic agents
May reduce the prokinetic effect of itopride. Avoid concurrent use or monitor for reduced efficacy.
Drugs metabolized by CYP3A4
Itopride is not significantly metabolized by CYP450 enzymes, so clinically significant interactions with CYP3A4 substrates are unlikely.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Limited information on overdose. Management should be symptomatic and supportive, including gastric lavage and close monitoring of vital signs.
Pregnancy & Lactation
Pregnancy Category B. Itopride should be used during pregnancy only if clearly needed and the potential benefits outweigh the potential risks. Excreted in animal milk; avoid use during breastfeeding or consult a doctor.
