Description
Overview of the medicine
Jakloc XR (Gliclazide Modified Release) is an oral antidiabetic medication used to control high blood sugar in people with type 2 diabetes mellitus. It belongs to the sulfonylurea class of drugs and works by stimulating the pancreas to produce more insulin.
Uses & Indications
Dosage
Adults
Initial dose of Gliclazide MR is typically 30 mg once daily, preferably with breakfast. Dose can be titrated up to a maximum of 120 mg once daily based on blood glucose response.
Elderly
Similar to adult dosage, but caution should be exercised due to increased risk of hypoglycemia; start with lower doses.
Renal_impairment
No dose adjustment required for mild to moderate renal impairment. Caution and careful monitoring are advised in severe renal impairment.
How to Take
Take orally once daily with breakfast. Swallow the tablet whole; do not crush, chew, or divide.
Mechanism of Action
Gliclazide primarily works by stimulating the beta cells of the islets of Langerhans in the pancreas to release insulin. It binds to the sulfonylurea receptor (SUR1) on the beta-cell membrane, leading to closure of ATP-sensitive potassium channels, depolarization, calcium influx, and subsequent insulin secretion.
Pharmacokinetics
Onset
Typically within a few hours for blood glucose lowering effect, peak plasma concentrations in 6-12 hours for MR.
Excretion
Mainly excreted in the urine (less than 1% as unchanged drug) and feces as metabolites.
Half life
Approximately 12-20 hours for MR formulation.
Absorption
Well absorbed orally. Modified release formulation ensures gradual and sustained release.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C9, to inactive metabolites.
Side Effects
Contraindications
- Type 1 Diabetes Mellitus
- Diabetic Ketoacidosis
- Severe renal or hepatic impairment
- Hypersensitivity to gliclazide or other sulfonylureas
- Concomitant use with miconazole
Drug Interactions
Miconazole (oral or systemic)
Potentiates hypoglycemic effect, leading to severe hypoglycemia. Concomitant use is contraindicated.
Corticosteroids, diuretics, sympathomimetics
May reduce the hypoglycemic effect.
Other antidiabetic agents (e.g., insulin, metformin)
Increased risk of hypoglycemia, requiring dose adjustment.
Beta-blockers, ACE inhibitors, MAO inhibitors, NSAIDs
May potentiate the hypoglycemic effect.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms include severe hypoglycemia. Management involves immediate glucose administration (oral or IV), monitoring blood glucose levels, and supportive care.
Pregnancy & Lactation
Not recommended during pregnancy (insulin is preferred). Not recommended during breastfeeding due to potential for hypoglycemia in the infant.

Doctor Notes
- Educate patients on hypoglycemia recognition and management.
- Regularly monitor renal and hepatic function, especially in elderly patients.
- Consider combination therapy if monotherapy is insufficient for glycemic control.
Patient Guidelines
- Take medicine at the same time each day with breakfast.
- Do not stop taking without consulting your doctor.
- Be aware of symptoms of hypoglycemia and carry a source of sugar.
- Regularly monitor blood glucose levels as advised by your doctor.
Missed Dose Advice
If a dose is missed, take it as soon as you remember, unless it's almost time for the next dose. Do not take a double dose to make up for a missed one.
Driving Precautions
Caution is advised as hypoglycemia may impair ability to drive or operate machinery. Patients should be aware of hypoglycemic symptoms and how to manage them before driving.
Lifestyle Advice
- Follow a balanced diet as recommended by your doctor or dietitian.
- Engage in regular physical activity.
- Maintain a healthy weight.
