Description
Overview of the medicine
Kabiven Perifer is a complete parenteral nutrition solution for adult patients unable to take food orally, enterally, or when oral/enteral nutrition is insufficient or contraindicated. It provides essential nutrients including amino acids, glucose, lipids, and electrolytes directly into the bloodstream.
Uses & Indications
Dosage
Adults
Dosage is individualized based on patient's nutritional requirements, weight, and metabolic state. Typically 14-28 ml/kg body weight/day, infused over 12-24 hours.
Elderly
Similar to adults, but careful monitoring is advised due to potential age-related physiological changes.
Renal_impairment
Dosage may need adjustment, especially for fluid volume and electrolyte content. Close monitoring of renal function and electrolyte levels is essential.
How to Take
Administer intravenously, preferably via a central venous catheter for prolonged use. Kabiven Perifer formulation is designed for peripheral venous administration due to its lower osmolality, but site rotation is recommended.
Mechanism of Action
The components of Kabiven Perifer are directly utilized by the body to provide energy, support protein synthesis, maintain electrolyte balance, and meet metabolic demands, bypassing the gastrointestinal tract.
Pharmacokinetics
Onset
Immediate, as components enter systemic circulation directly.
Excretion
Water, CO2, urea (from amino acid metabolism), and excess electrolytes are primarily excreted via the kidneys.
Half life
Varies for individual components; glucose, amino acids, and lipids are quickly utilized or metabolized by the body.
Absorption
Direct intravenous administration results in 100% bioavailability of all components.
Metabolism
Components are metabolized via normal physiological pathways (glucose for energy, amino acids for protein synthesis, lipids for energy/cellular structure).
Side Effects
Contraindications
- Hypersensitivity to egg, soy, or peanut protein, or to any active substances or excipients.
- Severe hyperlipidaemia.
- Severe liver insufficiency.
- Severe blood coagulation disorders.
- Severe renal insufficiency in patients without access to renal replacement therapy.
- Uncompensated diabetes mellitus.
- Acute shock.
- Pathologically elevated plasma concentrations of sodium, potassium, magnesium, calcium, and/or phosphate.
- Haemophagocytic syndrome.
Drug Interactions
Other intravenous drugs
Compatibility with other IV solutions/drugs must be checked before admixture to avoid precipitation or degradation.
Warfarin/Coumarin derivatives
Lipid emulsions may interfere with coagulation tests. Vitamin K in some TPN formulations can affect anticoagulant therapy.
Storage
Store below 25°C. Do not freeze. Store in outer carton to protect from light. Once mixed, use immediately or within 24 hours if refrigerated.
Overdose
Symptoms of overdose include fluid overload, electrolyte imbalances, hyperglycemia, hyperlipidemia, and metabolic acidosis. Management involves discontinuing the infusion immediately, correcting electrolyte/fluid imbalances, and providing supportive care.
Pregnancy & Lactation
Kabiven Perifer should be used during pregnancy and lactation only when clearly needed and the potential benefits outweigh the potential risks to the fetus or infant. Clinical supervision is essential.

Doctor Notes
- Strict aseptic technique is paramount during catheter insertion and maintenance to minimize infection risk.
- Individualize dosing based on patient's fluid status, metabolic needs, and laboratory parameters.
- Careful monitoring of blood glucose, electrolytes, liver and renal function is essential, especially during the initial phases of TPN.
- Educate healthcare team on recognizing and managing potential complications like refeeding syndrome and catheter-related bloodstream infections.
Patient Guidelines
- Kabiven Perifer is for hospital use under medical supervision.
- Patients should report any adverse effects or discomfort during infusion immediately.
- Regular blood tests will be performed to monitor nutrient levels and organ function.
Missed Dose Advice
As this medicine is administered in a clinical setting under professional supervision, missed doses are managed by healthcare providers according to the patient's treatment plan.
Driving Precautions
Not applicable, as Kabiven Perifer is administered in a clinical setting and often to patients who are not in a condition to drive.
