Overview of the medicine
Kanopas 20 mg tablet contains Pantoprazole, a proton pump inhibitor (PPI) used to treat conditions where there is too much acid in the stomach, such as gastroesophageal reflux disease (GERD), erosive esophagitis, and Zollinger-Ellison syndrome. It works by reducing the amount of acid your stomach makes.
Uses & Indications
Dosage
Adults
For GERD: 20-40 mg once daily for up to 8 weeks. For Erosive Esophagitis: 40 mg once daily for up to 8 weeks. For Zollinger-Ellison Syndrome: Initial dose 40 mg twice daily, adjusted based on response.
Elderly
No dosage adjustment generally required, but caution is advised in patients with severe hepatic impairment.
Renal_impairment
No dosage adjustment required for renal impairment or hemodialysis.
How to Take
Take orally, typically once daily, before a meal (preferably in the morning). Swallow the tablet whole with water; do not chew or crush it.
Mechanism of Action
Pantoprazole is a proton pump inhibitor that irreversibly binds to the H+/K+-ATPase enzyme system (the 'proton pump') in the gastric parietal cells, thereby inhibiting the final step in gastric acid secretion. This leads to a sustained decrease in gastric acid production.

Within 1 hour (antisecretory effect)
Excretion
Mainly excreted renally (approximately 80%) as metabolites, with the remainder excreted in feces.
Half life
Approximately 1 hour (terminal half-life), but the duration of acid suppression is much longer due to irreversible binding to the proton pump.
Absorption
Rapidly absorbed after oral administration; bioavailability is about 77%. Peak plasma concentrations are achieved in approximately 2 to 2.5 hours.
Metabolism
Extensively metabolized in the liver, primarily by the cytochrome P450 (CYP) enzyme system, mainly CYP2C19 and CYP3A4. Main metabolite is desmethylpantoprazole.
Side Effects
Contraindications
- Hypersensitivity to pantoprazole, other substituted benzimidazoles, or any component of the formulation.
- Patients with known severe liver disease may require caution.
Drug Interactions
Warfarin
Increased INR and prothrombin time, monitor carefully.
Clopidogrel
Potential reduction in clopidogrel's antiplatelet effect, consider alternative PPIs or antiplatelet therapy.
Methotrexate
Possible increased methotrexate levels, especially with high doses.
Drugs dependent on gastric pH (e.g., ketoconazole, atazanavir)
Reduced absorption due to increased gastric pH.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are generally mild and include lethargy, drowsiness, tachycardia, nausea, vomiting, and diarrhea. Treatment is symptomatic and supportive. Pantoprazole is not readily dialyzable.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Small amounts of pantoprazole have been detected in breast milk; a decision should be made whether to discontinue nursing or the drug, taking into account the importance of the drug to the mother.
