Description
Overview of the medicine
Keprad 250 mg Capsule contains Cephradine, a first-generation cephalosporin antibiotic used to treat various bacterial infections of the respiratory tract, urinary tract, skin, and soft tissues.
Uses & Indications
Dosage
Adults
Usually 250-500 mg every 6 hours or 500 mg-1 g every 12 hours. For severe infections, up to 1g every 6 hours.
Elderly
No specific dose adjustment typically required unless significant renal impairment is present.
Renal_impairment
Dosage reduction is necessary; e.g., CrCl 20-50 mL/min: 250 mg every 6 hours; CrCl <20 mL/min: 250 mg every 12-24 hours.
How to Take
Can be taken with or without food. Swallow the capsule whole with water. Do not chew or crush.
Mechanism of Action
Cephradine is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and death.
Pharmacokinetics
Onset
Within 30-60 minutes.
Excretion
Mainly by the kidneys via glomerular filtration and tubular secretion.
Half life
Approximately 0.7-2 hours (increased in renal impairment).
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract.
Metabolism
Minimal metabolism; primarily excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to cephalosporins or penicillins
- History of severe hypersensitivity reaction to any beta-lactam antibiotic
Drug Interactions
Probenecid
May increase Cephradine plasma levels and prolong its half-life by reducing renal excretion.
Anticoagulants (e.g., Warfarin)
May enhance the anticoagulant effect, increasing risk of bleeding. Monitor INR.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include nausea, vomiting, epigastric distress, and diarrhea. In severe cases, neurotoxicity (e.g., seizures) may occur, especially in patients with renal impairment. Management is primarily supportive; hemodialysis may be useful in severe cases.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but adequate and well-controlled studies in pregnant women are lacking. Use only if clearly needed. Excreted in breast milk in small amounts; use with caution in nursing mothers.
