Description
Overview of the medicine
Ketotifen is an antihistamine and mast cell stabilizer used for the prophylactic treatment of bronchial asthma, allergic rhinitis, allergic conjunctivitis, and other allergic conditions.
Uses & Indications
Dosage
Adults
1 mg twice daily with food. May be increased to 2 mg twice daily if necessary for severe cases, usually after 2-3 weeks.
Elderly
No specific dose adjustment is generally required, but caution is advised in elderly patients with hepatic or renal impairment.
Renal_impairment
No specific dose adjustment recommended, but use with caution and monitor for adverse effects.
How to Take
Administer orally with food to minimize gastrointestinal discomfort and enhance absorption. Take twice daily, preferably morning and evening.
Mechanism of Action
It acts as an H1-receptor antagonist and stabilizes mast cells, thereby inhibiting the release of inflammatory mediators like histamine and leukotrienes.
Pharmacokinetics
Onset
For asthma prophylaxis, full therapeutic effects may take several weeks to manifest (2-3 weeks). Acute allergic symptom relief may be faster.
Excretion
Mainly excreted renally as metabolites, with a small portion excreted unchanged.
Half life
The terminal elimination half-life is approximately 12 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract, with approximately 50% bioavailability due to first-pass metabolism.
Metabolism
Extensively metabolized in the liver, primarily via N-glucuronidation.
Side Effects
Contraindications
- Hypersensitivity to ketotifen or any components of the formulation.
- Patients with epilepsy or a history of seizures (due to potential lowering of seizure threshold).
- Concomitant use with oral antidiabetic agents (rare reports of reversible decrease in platelet count).
Drug Interactions
Oral Antidiabetics
Rare reports of reversible decrease in platelet count. Concomitant use should be avoided.
Anticholinergic Agents
May increase anticholinergic effects (e.g., dry mouth, urinary retention).
CNS Depressants (e.g., alcohol, sedatives, hypnotics, other antihistamines)
May potentiate the sedative effects.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include drowsiness, confusion, disorientation, tachycardia, hypotension, and possibly coma. Management is symptomatic and supportive. Gastric lavage may be considered in recent ingestions.
Pregnancy & Lactation
Pregnancy Category C. Use only if clearly needed and the potential benefits outweigh the potential risks. Not recommended during breastfeeding as ketotifen is excreted in breast milk.

Clinical Trials
Ketotifen has been extensively studied in clinical trials for its efficacy and safety in the long-term prophylactic treatment of bronchial asthma and other allergic conditions across various age groups.
Lab Monitoring
- No specific routine lab monitoring is required for short-term use. Periodic liver function tests (LFTs) may be considered during prolonged therapy, especially if liver impairment is suspected.
Doctor Notes
- Emphasize the prophylactic nature of Ketotifen for asthma; not for acute attacks.
- Advise patients about potential CNS depressant effects (drowsiness) and interactions with alcohol or other sedatives.
- Inform patients that full therapeutic effect for asthma may take several weeks.
Patient Guidelines
- Do not use for acute asthma attacks; it is a prophylactic medicine.
- Take consistently with food for best results.
- Be aware of potential drowsiness and avoid driving or operating heavy machinery until you know how the medicine affects you.
- Do not stop taking the medicine abruptly, especially if you have asthma, without consulting your doctor.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
