Description
Overview of the medicine
Ketotifen is an H1-antihistamine and mast cell stabilizer used for the long-term prophylactic treatment of bronchial asthma, allergic rhinitis, and allergic conjunctivitis. It is not effective for acute asthma attacks.
Uses & Indications
Dosage
Adults
1 mg (5 ml syrup) twice daily, morning and evening, with food. May be increased to 2 mg twice daily if necessary.
Elderly
No specific dosage adjustment for elderly patients, but caution is advised in those with impaired renal or hepatic function.
Renal_impairment
Use with caution in severe renal impairment; dose reduction may be necessary.
How to Take
Administer orally, preferably with food to minimize gastrointestinal discomfort. For children, use a measuring spoon or cup for accurate dosing.
Mechanism of Action
Ketotifen acts as a non-competitive H1-antihistamine and also stabilizes mast cells, thereby inhibiting the release of inflammatory mediators such as histamine, leukotrienes, and prostaglandins. It also possesses anti-anaphylactic properties and inhibits calcium influx in mast cells.
Pharmacokinetics
Onset
Antihistaminic effects appear relatively quickly, but prophylactic effects for asthma may take 2-4 weeks to become fully established.
Excretion
Approximately 60-70% excreted in urine and 30-40% in feces, mainly as metabolites.
Half life
Biphasic elimination with a terminal half-life of approximately 21 hours.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract. Bioavailability is about 50% due to first-pass metabolism.
Metabolism
Extensively metabolized in the liver, primarily via N-glucuronidation.

Drug Interactions
Oral Antidiabetics
There have been reversible decreases in platelet count reported rarely when ketotifen was given concomitantly with oral antidiabetic agents. Concomitant use should therefore be avoided.
Sedatives, Hypnotics, Antihistamines, Alcohol
Potentiation of their effects, leading to increased sedation.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms include drowsiness, confusion, disorientation, nystagmus, headache, tachycardia, and hypotension. Management involves symptomatic and supportive treatment, including gastric lavage or activated charcoal if ingestion is recent.
Pregnancy & Lactation
Pregnancy Category C. Should only be used during pregnancy if the potential benefit justifies the potential risk to the fetus. Ketotifen is excreted in breast milk; therefore, it should be avoided during lactation.
