Description
Overview of the medicine
Koftex 30 mg Syrup contains Dextromethorphan Hydrobromide, an antitussive agent used for the symptomatic relief of dry, unproductive cough caused by minor throat and bronchial irritation.
Uses & Indications
Dosage
Adults
Adults and children 12 years and over: 30 mg (5 mL) every 6-8 hours, not to exceed 120 mg (20 mL) in 24 hours.
Elderly
Same as adult dose; caution with impaired renal or hepatic function.
Renal_impairment
Use with caution. Dosage adjustment may be necessary in severe renal impairment.
How to Take
Administer orally with or without food. Use the provided measuring cup or spoon for accurate dosing.
Mechanism of Action
Dextromethorphan acts centrally on the cough center in the medulla oblongata to elevate the cough threshold, thereby suppressing the cough reflex.

Side Effects
Contraindications
- •Hypersensitivity to Dextromethorphan or any component of the syrup.
- •Patients currently taking or who have taken Monoamine Oxidase Inhibitors (MAOIs) within the last 14 days.
- •Patients with asthma, chronic bronchitis, or emphysema where cough is productive (with phlegm) unless directed by a doctor.
Drug Interactions
SSRIs, SNRIs, TCAs
Increased risk of serotonin syndrome.
Quinidine, Fluoxetine, Paroxetine
Inhibit CYP2D6, leading to increased Dextromethorphan levels and potential toxicity.
Monoamine Oxidase Inhibitors (MAOIs)
Concomitant use can lead to serotonin syndrome, characterized by hyperthermia, rigidity, mental status changes, and autonomic instability.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include nausea, vomiting, dizziness, drowsiness, blurred vision, nystagmus, ataxia, respiratory depression, and serotonin syndrome (in severe cases). Management involves supportive care and symptomatic treatment. Naloxone may be used in severe cases.
Pregnancy & Lactation
Pregnancy Category C. Use only if clearly needed and the potential benefits outweigh the potential risks to the fetus. Dextromethorphan is excreted in breast milk; consult a doctor before use during lactation.
