Description
Overview of the medicine
Vitamin K1 injection used to treat and prevent bleeding problems due to vitamin K deficiency, especially in newborns and in conditions like liver disease or anticoagulant overdose.
Uses & Indications
Dosage
Adults
For severe bleeding due to anticoagulants, 5-10 mg by slow IV injection; for less severe cases, 0.5-2 mg orally or IV. Dosage varies widely based on indication and severity.
Elderly
Similar to adults, but cautious dosing is advised, often starting at the lower end of the adult range.
Renal_impairment
No specific dose adjustment generally required, but caution is advised.
How to Take
Can be given by slow intravenous (IV), intramuscular (IM), or subcutaneous (SC) injection. IV route is preferred in emergencies.
Mechanism of Action
Phytomenadione is a fat-soluble vitamin K1 that is essential for the hepatic synthesis of active prothrombin (Factor II), proconvertin (Factor VII), plasma thromboplastin component (Factor IX), and Stuart factor (Factor X). It also participates in the carboxylation of various proteins.
Pharmacokinetics
Onset
1-2 hours (IV), 3-6 hours (IM/SC)
Excretion
Primarily via bile and urine, largely as glucuronide and sulfate conjugates.
Half life
Approximately 1.5-3 hours
Absorption
Rapidly absorbed after intramuscular or subcutaneous administration. Intravenous administration provides immediate systemic availability.
Metabolism
Rapidly metabolized in the liver to more polar metabolites.
Side Effects
Contraindications
- Hypersensitivity to phytomenadione or any component of the formulation.
- Severe liver impairment (relative contraindication due to decreased protein synthesis).
Drug Interactions
Salicylates (high doses)
May interfere with vitamin K action.
Oral anticoagulants (e.g., Warfarin)
Konakion reverses their effect; careful monitoring of INR is required.
Certain antibiotics (e.g., broad-spectrum antibiotics)
May reduce endogenous vitamin K production, increasing the need for exogenous vitamin K.
Storage
Store below 25°C. Protect from light. Do not freeze.
Overdose
Overdose is generally not associated with severe adverse effects, but excessive doses may interfere with anticoagulant therapy if it is subsequently required. Management is supportive.
Pregnancy & Lactation
Generally considered safe for use in pregnancy when indicated. Excreted in breast milk in small amounts; generally considered compatible with breastfeeding.

WHO Essential Medicine
YesClinical Trials
Widely studied and established, primarily observational studies on efficacy in various bleeding disorders and vitamin K deficiency. Established through extensive clinical use over decades.
Lab Monitoring
- Prothrombin time (PT) and International Normalized Ratio (INR) to monitor clotting status.
- Liver function tests if underlying liver disease is suspected.
Doctor Notes
- Administer slowly, especially IV, to minimize risk of anaphylactoid reactions.
- Check INR/PT before and after administration to confirm efficacy.
- Be aware of the different strengths available (1 mg vs 10 mg) to avoid dosing errors, especially in neonates.
Patient Guidelines
- Inform your doctor about all medications you are taking.
- Report any unusual bruising or bleeding.
- Avoid rapid IV administration as it can cause severe reactions.
Missed Dose Advice
Not typically applicable for an acute treatment like an injection for bleeding. Follow physician's instructions.
Driving Precautions
No known effects on the ability to drive or operate machinery.
Lifestyle Advice
- Maintain a balanced diet.
- Avoid sudden changes in dietary intake of vitamin K-rich foods if on warfarin.
