Overview of the medicine
Lacitab 2 mg Tablet contains Lacidipine, a dihydropyridine calcium channel blocker used to treat essential hypertension. It works by relaxing blood vessels, which helps to lower blood pressure.
Uses & Indications
Dosage
Adults
Initial dose: 2 mg once daily, preferably in the morning. May be increased to 4 mg after 3-4 weeks if needed. Max dose: 6 mg once daily.
Elderly
Initial dose: 2 mg once daily. Dose adjustment may be necessary based on clinical response and tolerability. Max dose: 6 mg once daily.
Renal_impairment
No dose adjustment is generally required for patients with renal impairment.
How to Take
Take the tablet orally once daily, preferably in the morning, with or without food. Swallow whole with water, do not chew or crush.
Mechanism of Action
Lacidipine selectively blocks L-type voltage-dependent calcium channels in vascular smooth muscle cells, leading to peripheral vasodilation and a reduction in systemic vascular resistance. This results in a decrease in blood pressure.

Onset
Typically within 30-60 minutes.
Excretion
Mainly excreted as inactive metabolites in feces (approx. 70%) and urine (approx. 30%).
Half life
Approximately 13-19 hours (terminal half-life), allowing for once-daily dosing.
Absorption
Rapidly absorbed from the gastrointestinal tract, but undergoes extensive first-pass metabolism. Absolute bioavailability is approximately 10-12%.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to lacidipine or any other dihydropyridine calcium channel blocker or to any excipients.
- •Cardiogenic shock.
- •Unstable angina pectoris.
- •Within one month of a myocardial infarction.
Drug Interactions
Beta-blockers
Concurrent use may lead to enhanced hypotensive effects or precipitation of cardiac failure in susceptible patients.
Diuretics and other antihypertensive agents
May increase the hypotensive effect of lacidipine.
Strong CYP3A4 inducers (e.g., Rifampicin, Phenytoin)
May significantly decrease plasma concentrations of lacidipine, reducing its efficacy.
Strong CYP3A4 inhibitors (e.g., Ketoconazole, Itraconazole, Ritonavir)
May significantly increase plasma concentrations of lacidipine, potentially leading to increased hypotensive effects. Concurrent use should be avoided or carefully monitored.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include marked and prolonged vasodilation, resulting in hypotension and reflex tachycardia. Treatment is supportive and symptomatic, including elevation of lower limbs, volume expansion with fluids, and if necessary, vasopressors.
Pregnancy & Lactation
Pregnancy Category C. Should only be used during pregnancy if the potential benefit outweighs the potential risk to the fetus. It is not recommended during lactation as lacidipine may be excreted in breast milk.
