Description
Overview of the medicine
Lactro-10 mg tablet is an osmotic laxative used to treat constipation and hepatic encephalopathy. It works by drawing water into the colon, which softens the stool and promotes bowel movements.
Uses & Indications
Dosage
Adults
Constipation: Initially 1-3 tablets daily. Maintenance dose adjusted to achieve 1-2 soft stools daily. Hepatic encephalopathy: Initially 3-4 tablets three to four times daily. Dosage adjusted to achieve 2-3 soft stools daily with a fecal pH of 5.0-5.5.
Elderly
Same as adult dosage, but may require lower initial dose and careful monitoring for electrolyte balance due to potential dehydration.
Renal_impairment
No specific dose adjustment required as it is minimally absorbed and primarily excreted unchanged in feces.
How to Take
Oral. Lactro-10 mg tablet can be taken with or without food. It may be swallowed whole with water. For better palatability, it can be mixed with water, fruit juice, or milk, if appropriate.
Mechanism of Action
Lactro-10 mg, containing an osmotic agent, increases the water content in the stool by drawing fluid into the colon. This softens the stool and stimulates peristalsis, promoting easier bowel movements.
Pharmacokinetics
Onset
Effect typically seen within 24-48 hours, as it requires bacterial metabolism in the colon.
Excretion
Primarily excreted unchanged in feces; small amounts (less than 3%) are absorbed and then excreted renally.
Half life
The half-life of unabsorbed lactulose is approximately 1.7-3 hours.
Absorption
Minimally absorbed from the gastrointestinal tract; less than 3% reaches the systemic circulation.
Metabolism
Not metabolized in the small intestine. Metabolized by colonic bacteria into lactic acid and short-chain fatty acids.
Side Effects
Contraindications
- •Galactosemia
- •Acute abdominal pain of unknown origin
- •Bowel obstruction
- •Hypersensitivity to lactulose or any component of the formulation
Drug Interactions
Antacids
May reduce the effect of lactulose, as lactulose requires an acidic colonic environment for its ammonia-reducing effect.
Other laxatives
Concurrent use may lead to excessive fluid and electrolyte loss.
Antibiotics (broad-spectrum)
May interfere with the action of lactulose by altering colonic bacterial flora, which are essential for its metabolism and therapeutic effect.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include abdominal pain, diarrhea, and electrolyte disturbances (especially hypokalemia and hyponatremia) due to excessive fluid loss. Treatment is supportive, involving fluid and electrolyte replacement. Discontinuation of the medicine or dose reduction may be necessary.
Pregnancy & Lactation
Lactro-10 mg is generally considered safe for use during pregnancy and lactation due to its minimal systemic absorption. However, like all medicines, use with caution and only under the advice of a healthcare professional.
