Description
Overview of the medicine
Lamitan 50 mg Tablet is an angiotensin II receptor blocker (ARB) used to treat high blood pressure (hypertension) and to help protect the kidneys from damage due to diabetes.
Uses & Indications
Dosage
Adults
The usual starting dose is 50 mg once daily. The dose can be increased to 100 mg once daily if needed, or divided into two doses.
Elderly
No initial dosage adjustment is typically required, but careful monitoring is advised.
Renal_impairment
No initial dosage adjustment is required for patients with mild to moderate renal impairment. For severe impairment (CrCl <30 mL/min), a lower starting dose (e.g., 25 mg) might be considered.
How to Take
Lamitan tablets can be taken with or without food. It is recommended to take the tablet at approximately the same time each day.
Mechanism of Action
Lamitan selectively blocks the binding of angiotensin II to the AT1 receptor found in many tissues (e.g., vascular smooth muscle, adrenal gland). This blockade prevents the vasoconstrictive and aldosterone-secreting effects of angiotensin II, leading to vasodilation, reduced blood pressure, and decreased fluid retention.
Pharmacokinetics
Onset
Within 6 hours
Excretion
Excreted primarily in feces (about 60%) and urine (about 35%).
Half life
Approximately 6-9 hours
Absorption
Well absorbed after oral administration, with about 25-33% bioavailability.
Metabolism
Undergoes hepatic metabolism, primarily via CYP2C9 and CYP3A4, to an active metabolite.
Side Effects
Contraindications
- •Hypersensitivity to Lamitan or any component of the formulation
- •Pregnancy (especially second and third trimesters)
- •Concomitant use with aliskiren in patients with diabetes or renal impairment (GFR <60 mL/min/1.73 m²)
Drug Interactions
Lithium
Increased serum lithium concentrations and toxicity have been reported.
NSAIDs (Nonsteroidal anti-inflammatory drugs)
May reduce the antihypertensive effect of Lamitan and increase the risk of renal impairment.
Potassium-sparing diuretics (e.g., spironolactone, amiloride) or potassium supplements
May lead to increased serum potassium levels (hyperkalemia).
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are likely to be hypotension and tachycardia; bradycardia could occur from parasympathetic stimulation. Management involves symptomatic and supportive treatment. If recently ingested, gastric lavage and/or activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy: Category D (2nd and 3rd trimesters). Can cause fetal harm. Discontinue immediately if pregnancy is detected. Lactation: Not recommended. It is unknown whether Lamitan is excreted in human milk.
