Description
Overview of the medicine
Insulin detemir is a long-acting insulin analog used to control high blood sugar in adults and children with diabetes mellitus. It provides a relatively constant glucose-lowering effect for up to 24 hours.
Uses & Indications
Dosage
Adults
Individualized based on patient's metabolic needs, blood glucose monitoring results, and glycemic control goal. Typically administered once or twice daily.
Elderly
Initiate with a lower dose and monitor closely due to potential for hypoglycemia and age-related decline in renal/hepatic function.
Renal_impairment
Dose reduction may be required. Monitor blood glucose closely.
How to Take
Administer subcutaneously once or twice daily at the same time(s) each day. Rotate injection sites within the same region (e.g., abdomen, thigh, upper arm). Do not administer intravenously or intramuscularly.
Mechanism of Action
Lowers blood glucose by stimulating peripheral glucose uptake, especially by skeletal muscle and fat, and by inhibiting hepatic glucose production. Insulin inhibits lipolysis and proteolysis, and enhances protein synthesis.
Pharmacokinetics
Onset
1-2 hours
Excretion
Primarily renal excretion of metabolites.
Half life
Approximately 5-9 hours, depending on dose and individual.
Absorption
Slow and protracted absorption from subcutaneous tissue due to multihexamer formation and reversible albumin binding.
Metabolism
Degraded by insulin-degrading enzymes similar to human insulin.
Side Effects
Contraindications
- During episodes of hypoglycemia.
- Hypersensitivity to insulin detemir or any of its excipients.
Drug Interactions
Beta-blockers
Mask symptoms of hypoglycemia and delay recovery.
Corticosteroids, Diuretics, Sympathomimetics
Decreased insulin effect, increased blood glucose.
Oral antidiabetic agents, ACE inhibitors, Aspirin
Increased risk of hypoglycemia.
Storage
Unopened: Store in refrigerator (2°C-8°C). Do not freeze. Opened/In-use: Store at room temperature (below 30°C) for up to 42 days. Protect from light and heat.
Overdose
Overdose can lead to severe hypoglycemia, which may be treated by oral glucose or intravenous glucose or glucagon depending on severity.
Pregnancy & Lactation
Use during pregnancy should be carefully considered, only if clearly needed. Insulin requirements may change during pregnancy and lactation.
