Overview of the medicine
Levetiracetam is an antiepileptic drug (AED) used in the treatment of various types of seizures. It is a pyrrolidine derivative that differs chemically from other AEDs.
Uses & Indications
Dosage
Adults
Initial dose 500 mg twice daily, increasing by 500 mg twice daily every 2 weeks to a maximum of 1500 mg twice daily. The dose can also be adjusted based on clinical response and tolerability.
Elderly
Initial dose may be lower, starting at 250 mg twice daily, with gradual titration. Dose adjustment needed in renal impairment.
Renal_impairment
Dosage should be individualized and adjusted based on creatinine clearance. For CrCl < 80 mL/min, lower doses and/or less frequent administration may be necessary.
How to Take
Take orally, with or without food. Tablets should be swallowed whole with a sufficient amount of liquid.
Mechanism of Action
The exact mechanism of action of levetiracetam is not fully understood, but it is thought to involve binding to the synaptic vesicle protein 2A (SV2A), which modulates neurotransmitter release and inhibits hypersynchronization of epileptiform burst firing.

Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Levetiracetam is excreted in breast milk; a decision should be made whether to discontinue nursing or discontinue the drug, taking into account the importance of the drug to the mother.
