Description
Overview of the medicine
Levetam-XR 500 mg Tablet is an extended-release antiepileptic medicine containing Levetiracetam, used to control various types of seizures in epilepsy.
Uses & Indications
Dosage
Adults
Initial dose is typically 1000 mg once daily (two 500 mg tablets). Dosage may be increased by 500 mg or 1000 mg once daily every 2 weeks to a maximum of 3000 mg once daily.
Elderly
No specific dose adjustment is required based solely on age, but dose adjustments may be necessary due to age-related decline in renal function.
Renal_impairment
Dosage reduction is recommended for patients with impaired renal function. Consult physician for specific dose adjustments based on creatinine clearance.
How to Take
Take Levetam-XR 500 mg Tablet orally, once daily. Swallow the tablet whole; do not chew, crush, or break it. It can be taken with or without food. Take it at the same time each day.
Mechanism of Action
Levetiracetam binds to the synaptic vesicle protein 2A (SV2A) in the brain, which is thought to modulate neurotransmitter release and thus exert its anticonvulsant effects.
Pharmacokinetics
Onset
Varies by individual; XR formulation maintains steady plasma levels for once-daily dosing.
Excretion
Mainly renal, with 66% excreted unchanged in urine.
Half life
Approximately 7-8 hours; prolonged in elderly and renally impaired patients.
Absorption
Rapidly and almost completely absorbed. Extended-release formulation provides sustained plasma concentrations.
Metabolism
Not extensively metabolized by hepatic CYP enzymes. Primarily by enzymatic hydrolysis of the acetamide group.
Side Effects
Contraindications
- Hypersensitivity to levetiracetam or any component of the formulation.
Drug Interactions
Methotrexate
Concomitant use with methotrexate may increase methotrexate concentrations, potentially requiring dose adjustment.
Central Nervous System Depressants
Concurrent use with alcohol or other CNS depressants may increase somnolence and dizziness.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, agitation, aggression, depressed level of consciousness, respiratory depression, and coma. Management is supportive and symptomatic, including gastric lavage or induced emesis.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Levetiracetam is excreted into breast milk; caution should be exercised when administered to a nursing woman.
