Description
Overview of the medicine
Leviceta 500 mg Oral Solution is an antiepileptic drug containing Levetiracetam, used to control various types of seizures in patients with epilepsy.
Uses & Indications
Dosage
Adults
Initial dose: 500 mg twice daily. Can be increased by 500 mg twice daily increments every 2 weeks to a maximum of 1500 mg twice daily.
Elderly
No specific dose adjustment is required based on age alone. Dose adjustment needed for renal impairment.
Renal_impairment
Dose adjustment necessary based on creatinine clearance. For CrCl 50-79 mL/min: 500-1000 mg twice daily; for CrCl 30-49 mL/min: 250-750 mg twice daily; for CrCl <30 mL/min: 250-500 mg twice daily.
How to Take
Take orally, typically twice daily, with or without food. Use the provided measuring device to ensure accurate dosing.
Mechanism of Action
The exact mechanism is unknown, but it is believed to involve binding to synaptic vesicle protein 2A (SV2A) in the brain, thereby modifying synaptic transmission and neuronal excitability to prevent seizures.
Pharmacokinetics
Onset
Within 1-2 hours
Excretion
Excreted predominantly in the urine, with about 69% of the dose eliminated unchanged.
Half life
Approximately 7 hours in adults, may be longer in children and elderly with impaired renal function.
Absorption
Rapidly and almost completely absorbed after oral administration. Peak plasma concentrations are achieved within 1 hour.
Metabolism
Minimal metabolism in humans, primarily by enzymatic hydrolysis of the acetamide group, not via CYP450 system.
Side Effects
Contraindications
- Hypersensitivity to Levetiracetam or any component of the formulation.
- Severe hepatic impairment (use with caution).
Drug Interactions
Methotrexate
Possible decrease in methotrexate clearance, leading to increased methotrexate levels and potential toxicity (rare).
Central Nervous System (CNS) Depressants
Concurrent use may increase CNS depression (e.g., drowsiness, dizziness). Caution with alcohol, opioids, benzodiazepines.
Storage
Store at room temperature (below 25°C), away from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, agitation, aggression, depressed level of consciousness, respiratory depression, and coma. Management is supportive, including gastric lavage or activated charcoal if recent ingestion, and close monitoring of vital signs. Hemodialysis can remove Levetiracetam.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Excreted in breast milk; caution should be exercised when administered to a nursing woman.

Lab Monitoring
- Renal function tests (creatinine clearance) for dose adjustment, especially in elderly and renally impaired patients.
- Complete blood count (CBC) in rare cases if symptoms of blood dyscrasias appear.
Doctor Notes
- Monitor patients for signs of behavioral changes, depression, or suicidal ideation, especially during initial therapy or dose adjustments.
- Adjust dose in patients with impaired renal function based on creatinine clearance. Gradual titration is recommended to minimize side effects.
Patient Guidelines
- Do not stop taking this medicine abruptly, as it may increase the frequency of seizures. Always consult your doctor before making any changes.
- Take your medicine regularly at the same times each day for optimal control of seizures.
- Report any unusual mood changes, depression, or suicidal thoughts to your doctor immediately.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is close to the time for your next dose, skip the missed dose and continue with your regular schedule. Do not take a double dose to make up for a missed one.
