Leviram 250 mg tablet contains Levetiracetam, an antiepileptic drug used to treat various types of seizures in individuals with epilepsy. It helps to reduce the frequency and severity of seizures.
Uses & Indications
Dosage
Adults
Initial dose 500 mg twice daily, may be increased up to 1500 mg twice daily. Dosage adjustments should be made in increments of 500 mg twice daily every 2 weeks.
Elderly
No specific dose adjustments needed for elderly patients with normal renal function. Dose adjustments may be necessary based on renal function.
Renal_impairment
Dosage must be adjusted in patients with impaired renal function. Consult a physician for specific recommendations based on creatinine clearance.
How to Take
Leviram tablets should be swallowed whole with a sufficient amount of liquid. It can be taken with or without food. Do not chew, crush, or break the tablet.
Mechanism of Action
The exact mechanism of action of Levetiracetam is not fully understood, but it is believed to exert its antiepileptic effects by binding to the synaptic vesicle protein 2A (SV2A) in the brain, thereby modulating neurotransmitter release and neuronal excitability.
Pharmacokinetics
Onset
Within 1 hour for oral solution, somewhat longer for tablets but effects generally seen within days of stable dosing.
Excretion
Primarily excreted unchanged in the urine (approximately 66% of the dose), with a small amount of the inactive metabolite also excreted renally.
Half life
Approximately 6-8 hours in adults, longer in elderly and renally impaired patients.
Absorption
Rapidly and almost completely absorbed after oral administration, with peak plasma concentrations occurring within 1 hour.
Metabolism
Not extensively metabolized in humans; undergoes enzymatic hydrolysis of the acetamide group to form an inactive carboxylic acid derivative.
Side Effects
Contraindications

Drug Interactions
Methotrexate
Decreased renal clearance of methotrexate leading to increased methotrexate plasma concentration.
CNS Depressants
Concurrent use with other CNS depressants (e.g., alcohol, opioids, sedatives) may increase somnolence and dizziness.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, agitation, aggression, depressed level of consciousness, respiratory depression, and coma. Treatment should be symptomatic and supportive, including gastric lavage or activated charcoal if appropriate. Hemodialysis may be used.
Pregnancy & Lactation
Pregnancy Category C. Studies in animals have shown adverse effects on the fetus. Levetiracetam is excreted in human breast milk. Consult a doctor before use during pregnancy or lactation. The decision to breastfeed should consider the potential for adverse reactions in the infant.
