Overview of the medicine
Levoday 500 mg Tablet contains Levofloxacin, a synthetic broad-spectrum antibacterial agent of the fluoroquinolone class, used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
Usually 500 mg once daily for 7-14 days depending on the infection. For some severe infections, 750 mg once daily may be used.
Elderly
No specific dose adjustment is required based on age alone, but renal function should be assessed.
Renal_impairment
Dosage adjustment is necessary for patients with creatinine clearance <50 mL/min. For example, 500 mg initial dose, then 250 mg every 24-48 hours depending on creatinine clearance.
How to Take
Take orally with or without food. Do not take with antacids containing magnesium or aluminum, sucralfate, iron, or zinc supplements; administer at least 2 hours before or 2 hours after these agents.
Mechanism of Action
Levofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes necessary for bacterial DNA replication, transcription, repair, and recombination, thereby leading to bacterial cell death.

Onset
Peak plasma concentrations reached within 1-2 hours after oral administration.
Excretion
Primarily excreted unchanged in the urine, mainly through glomerular filtration and tubular secretion.
Half life
Approximately 6-8 hours, allowing for once-daily dosing.

Rapid and complete oral absorption, bioavailability approximately 99%.
Metabolism
Minimally metabolized in the liver (less than 5% of a dose).

Contraindications
- Hypersensitivity to levofloxacin, other quinolones, or any component of the formulation.
- History of tendinitis or tendon rupture associated with fluoroquinolone use.
- Epilepsy or other seizure disorders.
Drug Interactions
NSAIDs
May increase the risk of CNS stimulation and convulsive seizures.
Warfarin
May enhance the anticoagulant effect of warfarin, increasing bleeding risk.
Corticosteroids
Increased risk of tendinopathy and tendon rupture.
Antacids (Mg/Al), Sucralfate, Iron/Zinc supplements
Reduce oral absorption of levofloxacin.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.

In case of overdose, symptoms may include CNS effects (confusion, dizziness, seizures), QT prolongation. Treatment is symptomatic and supportive. Gastric lavage and adequate hydration may be helpful. Hemodialysis is not effective.
