Description
Overview of the medicine
Levomax-TS 0.5% Eye Drop is an antibiotic medication containing Levofloxacin, a fluoroquinolone antibiotic. It is used to treat bacterial infections of the eye, such as bacterial conjunctivitis and corneal ulcers, by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
For bacterial conjunctivitis: Instill 1-2 drops into the affected eye(s) every 2 hours while awake, up to 8 times a day, for the first 2 days. Then, 1-2 drops every 4 hours while awake, up to 4 times a day, for the next 5 days or as directed by physician. For corneal ulcers: Instill 1-2 drops into the affected eye(s) every 15-30 minutes for the first few hours, then every 1 hour, then gradually reduce frequency over 7-14 days.
Elderly
No specific dosage adjustment is generally required for elderly patients.
Renal_impairment
No specific dosage adjustment is necessary for ophthalmic use as systemic absorption is minimal.
How to Take
For ophthalmic use only. Wash hands thoroughly before administration. Tilt head back, pull down the lower eyelid to form a pouch, instill the drops into the pouch, and gently close the eye for a few minutes. Avoid touching the dropper tip to any surface (eye, finger, or other) to prevent contamination. If more than one ophthalmic medication is used, wait at least 5 minutes between applications to ensure proper absorption.
Mechanism of Action
Levofloxacin, a third-generation fluoroquinolone, inhibits bacterial DNA gyrase (topoisomerase II) and topoisomerase IV. These enzymes are crucial for bacterial DNA replication, transcription, repair, and recombination. By inhibiting them, Levofloxacin causes bacterial cell death, thus exerting its bactericidal effect.

Onset
Within hours of administration, with symptomatic improvement typically observed within 2-5 days.
Excretion
Primarily excreted unchanged in urine following systemic absorption, but locally in tears and ocular surface.
Half life
Plasma half-life is approximately 6-8 hours, but ocular tissue half-life varies. Excretion from ocular surface is rapid.
Absorption
Following topical ophthalmic administration, levofloxacin is absorbed into the ocular tissues. Systemic absorption is minimal.
Metabolism
Minimally metabolized. Less than 5% of an orally administered dose is metabolized.

Systemic Quinolones
Although systemic absorption is minimal with topical ophthalmic use, theoretical additive effects with systemic quinolones are possible but not clinically significant under normal usage.
Other Ophthalmic Medications
If using other eye drops, administer them at least 5 minutes apart to avoid washout and ensure adequate drug contact time with the eye.
Storage
Store at controlled room temperature (20°C to 25°C), excursions permitted to 15°C to 30°C. Protect from light. Do not freeze. Keep the bottle tightly closed when not in use.
Overdose
Overdose is unlikely with topical ophthalmic administration. If an accidental overdose occurs, flush the eye with lukewarm water. Symptomatic treatment may be initiated if necessary.
