Description
Overview of the medicine
Levomet is a brand name for Levofloxacin, a synthetic broad-spectrum antibacterial agent of the fluoroquinolone class. It is used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
Typically 500 mg once daily for 7-14 days, depending on the infection type and severity. For some severe infections, 750 mg once daily.
Elderly
No dose adjustment required based on age alone, but renal function should be assessed.
Renal_impairment
Dosage adjustment is necessary for patients with creatinine clearance < 50 mL/min.
How to Take
Levomet tablets can be taken orally with or without food. It should be taken with a full glass of water. Do not crush or chew the tablet.
Mechanism of Action
Levofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes necessary for bacterial DNA replication, transcription, repair, and recombination, leading to bacterial cell death.
Pharmacokinetics
Onset
Antibacterial effect typically observed within hours of first dose.
Excretion
Primarily excreted unchanged in the urine, mainly by glomerular filtration and tubular secretion.
Half life
Approximately 6-8 hours, allowing for once-daily dosing.
Absorption
Rapid and nearly complete oral absorption, with peak plasma concentrations achieved within 1-2 hours.
Metabolism
Minimally metabolized in the liver; approximately 5% of a dose is metabolized.
Side Effects
Contraindications
- Hypersensitivity to levofloxacin, other fluoroquinolones, or any component of the formulation
- History of tendon disorders related to fluoroquinolone administration
- Epilepsy or other seizure disorders
Drug Interactions
0
Antacids containing magnesium or aluminum, sucralfate, metal cations (iron, zinc): May reduce absorption of levofloxacin. Administer levofloxacin at least 2 hours before or 2 hours after these agents.
1
Warfarin: May enhance the anticoagulant effects of warfarin. Monitor INR.
2
Corticosteroids: Concomitant use increases the risk of tendon disorders.
3
NSAIDs: May increase the risk of CNS stimulation and convulsive seizures.
4
Anti-arrhythmic drugs (e.g., Class IA and III antiarrhythmics, tricyclic antidepressants): Increased risk of QT prolongation.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
In the event of an acute overdose, the stomach should be emptied. The patient should be observed and given symptomatic and supportive treatment. Hydration should be maintained. Hemodialysis or peritoneal dialysis is not effective in removing levofloxacin from the body.
Pregnancy & Lactation
Pregnancy Category C. Levofloxacin should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk; therefore, a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
