Overview of the medicine
Levonix (Levofloxacin) is a synthetic broad-spectrum antibacterial agent of the fluoroquinolone class. It is used to treat a wide range of bacterial infections including respiratory tract, urinary tract, skin, and soft tissue infections.
Uses & Indications
Dosage
Adults
Typically 250 mg to 750 mg once daily, depending on the infection. For most infections, 500 mg once daily for 7-14 days.
Elderly
No specific dose adjustment needed based on age alone, but renal function should be assessed.
Renal_impairment
Dose adjustment required based on creatinine clearance. For CrCl 20-49 mL/min: Initial 500 mg, then 250 mg every 24 hours. For CrCl <20 mL/min: Initial 500 mg, then 250 mg every 48 hours.
How to Take
May be taken with or without food. Ensure adequate fluid intake. Do not take with antacids containing magnesium or aluminum, sucralfate, iron or zinc supplements.
Mechanism of Action
Levofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes for bacterial DNA replication, transcription, repair, and recombination. This leads to bacterial cell death.

Onset
Within 1-2 hours for therapeutic effect.
Excretion
Primarily excreted unchanged in the urine (approximately 87% within 48 hours).
Half life
Approximately 6-8 hours.
Absorption
Rapidly and almost completely absorbed after oral administration. Peak plasma concentrations are achieved within 1-2 hours.
Metabolism
Minimally metabolized in the liver (less than 5% of a dose).

Contraindications
- Hypersensitivity to Levofloxacin or other fluoroquinolones
- History of tendinitis or tendon rupture associated with fluoroquinolone use
- Epilepsy
- Children and adolescents (under 18 years)
Drug Interactions
NSAIDs
May increase risk of CNS stimulation and convulsive seizures.
Warfarin
May enhance anticoagulant effects.
Corticosteroids
Increased risk of tendon rupture.
Antiarrhythmics (e.g., Quinidine, Procainamide)
Increased risk of QT prolongation.
Antacids (Magnesium/Aluminum), Sucralfate, Iron/Zinc Supplements
Reduce absorption of Levofloxacin.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include CNS symptoms (confusion, tremors, seizures), QT prolongation. Management includes symptomatic and supportive measures. Gastric lavage and hydration may be helpful. Hemodialysis is not effective.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Excreted into breast milk, so breastfeeding is not recommended during treatment.
