Description
Overview of the medicine
Lexa (Escitalopram) is an antidepressant medication primarily used to treat major depressive disorder and generalized anxiety disorder. It works by restoring the balance of a natural substance (serotonin) in the brain.
Uses & Indications
Dosage
Adults
Major Depressive Disorder and Generalized Anxiety Disorder: Initial dose 10 mg once daily, may be increased to a maximum of 20 mg once daily after at least one week. Panic Disorder, Social Anxiety Disorder, OCD: Initial dose 5 mg once daily, may be increased to 10 mg after one week, maximum 20 mg once daily.
Elderly
Recommended initial dose 5 mg once daily, maximum 10 mg once daily.
Renal_impairment
No dosage adjustment needed for mild to moderate impairment. Use with caution in severe renal impairment.
How to Take
Take orally once daily with or without food. Swallow the tablet whole; do not chew, crush, or break.
Mechanism of Action
Escitalopram selectively inhibits the reuptake of serotonin (5-HT) in the brain, thereby increasing the concentration of serotonin in the synaptic cleft. This leads to enhanced serotonergic neurotransmission.
Pharmacokinetics
Onset
Initial effects may be seen in 1-2 weeks, full antidepressant effect in 4-6 weeks.
Excretion
Primarily via urine as metabolites.
Half life
Approximately 27-32 hours.
Absorption
Well absorbed orally, peak plasma concentrations reached in about 3-4 hours.
Metabolism
Extensively metabolized in the liver by CYP2C19, CYP3A4, and CYP2D6 enzymes to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to escitalopram or citalopram.
- Concomitant use of Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing an MAOI, or starting an MAOI within 14 days of discontinuing escitalopram.
- Congenital long QT syndrome or concomitant use with other QT-prolonging medicinal products.
Drug Interactions
MAOIs
Risk of serious, sometimes fatal, reactions including serotonin syndrome.
Triptans
Increased risk of serotonin syndrome.
NSAIDs/Aspirin
Increased risk of gastrointestinal bleeding.
Warfarin and other anticoagulants
Altered anticoagulant effects; monitor INR.
Other serotonergic drugs (e.g., tramadol, St. John's Wort)
Increased risk of serotonin syndrome.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms include dizziness, tremor, somnolence, nausea, vomiting, tachycardia, and convulsions. Management involves supportive measures, airway maintenance, and activated charcoal. There is no specific antidote.
Pregnancy & Lactation
