Overview of the medicine
Lexopam (Bromazepam) is a benzodiazepine derivative with anxiolytic, sedative, muscle relaxant, and anticonvulsant properties. It is primarily used for the short-term treatment of anxiety, tension, and related somatic or psychiatric conditions, as well as insomnia.
Uses & Indications
Dosage
Adults
Typical daily dose is 3-18 mg, divided into 2-3 doses. In severe cases, up to 30 mg daily. Treatment should be as short as possible.
Elderly
Initially, a lower dose of 1.5 mg once or twice daily. Gradually increase if needed and tolerated.
Renal_impairment
Use with caution and consider dose reduction; monitor for adverse effects.
How to Take
Tablets should be swallowed whole with water. Can be taken with or without food. To be used for short-term treatment only.
Mechanism of Action
Bromazepam enhances the effect of gamma-aminobutyric acid (GABA), an inhibitory neurotransmitter, at the GABAA receptor in the central nervous system. This leads to increased chloride ion influx, hyperpolarization of the neuron, and decreased neuronal excitability, resulting in anxiolytic, sedative, and muscle relaxant effects.

Mainly excreted in the urine as metabolites.

Approximately 10-20 hours.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract; peak plasma concentrations achieved within 1-2 hours.

Extensively metabolized in the liver, primarily by CYP3A4, into inactive metabolites.
Side Effects
Contraindications

Drug Interactions
CYP3A4 Inducers (e.g., rifampicin, carbamazepine)
May decrease bromazepam plasma concentrations and reduce its effects.
CYP3A4 Inhibitors (e.g., ketoconazole, erythromycin)
May increase bromazepam plasma concentrations and enhance its effects.
CNS Depressants (e.g., alcohol, opioids, antidepressants)
Increased sedative effect, respiratory depression, hypotension.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms include drowsiness, confusion, ataxia, diminished reflexes, hypotension, respiratory depression, and coma. Management involves supportive care, maintaining airway, monitoring vital signs, and if severe, administration of flumazenil (a benzodiazepine receptor antagonist).
Pregnancy & Lactation
Pregnancy Category D. Not recommended during pregnancy, especially in the first trimester, due to potential risks of congenital malformations. Avoid during lactation as it passes into breast milk and may cause sedation in the infant.
