Description
Overview of the medicine
Lifecal-D 500 mg tablet is a combination of Calcium Carbonate and Vitamin D3, used to prevent and treat calcium and vitamin D deficiencies. It plays a crucial role in maintaining bone health and various metabolic functions.
Uses & Indications
Dosage
Adults
Typically 1-2 tablets daily, or as directed by the physician.
Elderly
Same as adult dosage, but caution with renal function. Physician consultation is recommended.
Renal_impairment
Use with caution and dose adjustment may be necessary due to risk of hypercalcemia. Avoid in severe renal impairment.
How to Take
Swallow the tablet whole with water, preferably with or after meals to enhance absorption and minimize gastrointestinal upset. Do not chew or crush.
Mechanism of Action
Calcium is an essential mineral required for bone formation, muscle contraction, nerve function, and blood coagulation. Vitamin D3 (Cholecalciferol) is crucial for regulating calcium and phosphate homeostasis, promoting calcium absorption from the intestine, and facilitating bone mineralization. Together, they help maintain healthy bones and prevent disorders related to mineral deficiency.
Pharmacokinetics
Onset
Bone mineral density improvement observed over weeks to months with consistent use.
Excretion
Calcium is primarily excreted via feces (unabsorbed) and urine (excess absorbed). Vitamin D metabolites are excreted mainly in bile/feces and some in urine.
Half life
Calcium: Variable, related to bone turnover. Vitamin D3: Approximately 15-20 days for calcifediol (25-hydroxyvitamin D3), but active form calcitriol has shorter half-life.
Absorption
Calcium absorption is primarily in the small intestine, enhanced by Vitamin D. Vitamin D3 is absorbed from the small intestine, mainly in the jejunum, and transported via lymphatic system.
Metabolism
Calcium is not metabolized. Vitamin D3 is hydroxylated in the liver to 25-hydroxyvitamin D3 (calcifediol) and then in the kidneys to 1,25-dihydroxyvitamin D3 (calcitriol), the active form.
Side Effects
Contraindications
- •Hypersensitivity to the active substances or to any of the excipients.
- •Hypercalcemia (abnormally high levels of calcium in the blood).
- •Severe hypercalciuria (excessive calcium in urine).
- •Severe renal impairment or renal calculus (kidney stones).
- •Primary hyperparathyroidism.
- •Vitamin D intoxication.
Drug Interactions
Corticosteroids
Can reduce calcium absorption.
Cardiac Glycosides
Hypercalcemia can potentiate the effects of cardiac glycosides, increasing the risk of arrhythmias.
Thiazide Diuretics
May increase the risk of hypercalcemia by decreasing urinary calcium excretion.
Bisphosphonates/Levothyroxine
Calcium can impair absorption of these drugs. Administer at least 4 hours apart.
Tetracyclines/Fluoroquinolones
Calcium can reduce the absorption of these antibiotics. Administer at least 2-3 hours apart.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypercalcemia, nausea, vomiting, constipation, thirst, polyuria, anorexia, weakness, and fatigue. Severe cases can lead to renal impairment and cardiac arrhythmias. Management involves discontinuing the product, hydration, and medical supervision. In severe cases, corticosteroids or calcitonin may be used.
Pregnancy & Lactation
Lifecal-D is generally safe and often recommended during pregnancy and lactation to meet increased calcium and Vitamin D requirements. However, doses should not exceed recommended limits to avoid hypercalcemia in the mother and fetus/infant. Consult a physician before use.
