Lijenta 5 mg Tablet contains Linagliptin, an oral medicine used to help control blood sugar levels in adults with type 2 diabetes mellitus. It is used as an adjunct to diet and exercise.
Uses & Indications
Dosage
Adults
The recommended dose is 5 mg orally once daily, with or without food.
Elderly
No dosage adjustment required based on age.
Renal_impairment
No dosage adjustment required in patients with renal impairment, including those on dialysis.
How to Take
Administer orally once daily, at any time of day, with or without food.
Mechanism of Action
Linagliptin inhibits the enzyme dipeptidyl peptidase-4 (DPP-4), which normally degrades incretin hormones (GLP-1 and GIP). By inhibiting DPP-4, linagliptin increases the levels of active incretin hormones, leading to increased insulin synthesis and release from pancreatic beta cells and decreased glucagon secretion from pancreatic alpha cells. This results in glucose-dependent lowering of blood sugar.

Onset
Glycemic control observed within a few days of initiation.
Excretion
Primarily excreted unchanged via bile/faeces (about 80%), with a smaller portion (about 5%) excreted via urine.
Half life
Apparent terminal half-life is >100 hours, but effective half-life for DPP-4 inhibition is about 12 hours.
Absorption
Rapidly absorbed. Peak plasma concentration (Cmax) reached within 1.5 hours. Absolute bioavailability is approximately 30%.
Metabolism
Limited metabolism; primarily CYP3A4-mediated minor metabolism (approximately 10%).
Side Effects
Contraindications
- Hypersensitivity to linagliptin or any component of the formulation.
- Patients with type 1 diabetes mellitus or for the treatment of diabetic ketoacidosis.
Drug Interactions
Rifampin
Decreases linagliptin exposure, potentially reducing efficacy. Avoid coadministration if possible.
Insulin/Sulfonylureas
May increase risk of hypoglycemia; a lower dose of insulin or sulfonylurea may be required.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, no specific antidote is known. Symptomatic and supportive treatment should be initiated as appropriate for the patient's clinical state.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. It is not known whether linagliptin is excreted in human milk; use with caution in nursing mothers.
