Description
Overview of the medicine
Locaine 2% Injection is a sterile solution containing Lidocaine Hydrochloride, an amide-type local anesthetic, used for infiltration, nerve block, epidural, and spinal anesthesia. It temporarily blocks nerve impulses, providing localized pain relief.
Uses & Indications
Dosage
Adults
Varies widely based on procedure, site, and individual response. Max single dose 4.5 mg/kg (or 300 mg total) without epinephrine, 7 mg/kg (or 500 mg total) with epinephrine.
Elderly
Reduce dose, monitor carefully.
Renal_impairment
Use with caution, dose adjustment may be necessary.
How to Take
Administered by injection, typically subcutaneously, intramuscularly, or intravenously depending on the type of anesthesia or procedure. Always administered by a trained healthcare professional.
Mechanism of Action
Lidocaine stabilizes the neuronal membrane by inhibiting the ionic fluxes required for the initiation and conduction of impulses, thereby effecting local anesthetic action. It blocks voltage-gated sodium channels.
Pharmacokinetics
Onset
2-5 minutes (infiltration), 5-15 minutes (nerve block)
Excretion
Primarily via kidneys as metabolites and <10% as unchanged drug.
Half life
1.5-2 hours (in adults)
Absorption
Rapidly absorbed from the site of injection. Rate and extent depend on vascularity of the tissue.
Metabolism
Primarily in the liver by microsomal enzymes (oxidative N-dealkylation).
Side Effects
Contraindications
- •Hypersensitivity to lidocaine or other amide-type local anesthetics
- •Adam-Stokes syndrome
- •Severe degrees of heart block
- •Wolff-Parkinson-White (WPW) syndrome
- •Uncontrolled epilepsy
Drug Interactions
Cimetidine
May inhibit lidocaine metabolism, increasing plasma levels.
Beta-blockers
May decrease hepatic metabolism of lidocaine, increasing risk of toxicity.
Succinylcholine
Prolonged neuromuscular blockade.
Class I Antiarrhythmics (e.g., mexiletine, procainamide)
Additive cardiac effects.
Storage
Store below 30°C. Protect from light. Do not freeze.
Overdose
Symptoms include CNS excitation (seizures, tremors), followed by CNS depression (respiratory depression, coma), and cardiovascular effects (hypotension, bradycardia, cardiac arrest). Management involves maintaining airway, administering oxygen, managing seizures (e.g., with benzodiazepines), and supporting cardiovascular function. Intravenous lipid emulsion therapy may be used for severe cases.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Lidocaine is excreted into breast milk; use with caution in nursing mothers.
