Overview of the medicine
Lodipin 5 mg Tablet contains Amlodipine, a calcium channel blocker used to treat high blood pressure (hypertension) and chest pain (angina). It works by relaxing blood vessels, allowing blood to flow more easily.
Uses & Indications
Dosage
Adults
Initial dose 5 mg once daily; maximum 10 mg once daily. Dosage should be adjusted according to patient response and tolerance.
Elderly
No specific dose adjustment is typically required, but use with caution. Initial dose 2.5 mg once daily may be considered.
Renal_impairment
No dosage adjustment is required for renal impairment.
How to Take
Oral. Can be taken with or without food. Take at the same time each day.
Mechanism of Action
Amlodipine is a dihydropyridine calcium channel blocker that inhibits the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle. This relaxation of vascular smooth muscle leads to vasodilation and reduction in peripheral vascular resistance, thus lowering blood pressure. In angina, it reduces myocardial oxygen demand by dilating peripheral arterioles and epicardial coronary arteries.
Pharmacokinetics
Onset
Onset of antihypertensive effect is gradual, usually within 24 hours.
Excretion
Excreted primarily via the urine (60% as metabolites, 10% as unchanged drug) and feces (20-25%).
Half life
30-50 hours (terminal elimination half-life)
Absorption
Well absorbed after oral administration, bioavailability is 64-90%. Peak plasma concentrations are reached 6-12 hours after dosing. Food does not affect absorption.
Metabolism
Extensively metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- Known hypersensitivity to amlodipine or other dihydropyridines.
- Severe hypotension.
- Cardiogenic shock.
- Clinically significant aortic stenosis.
Drug Interactions
Simvastatin
Amlodipine may increase simvastatin exposure, limit simvastatin dose to 20 mg/day.
Clarithromycin
Increased risk of hypotension and acute kidney injury.
CYP3A4 Inducers (e.g., rifampicin, St. John's Wort)
May decrease amlodipine exposure, monitor blood pressure.
CYP3A4 Inhibitors (e.g., ketoconazole, itraconazole, ritonavir)
May increase amlodipine exposure, monitor for hypotension.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.

Symptoms include excessive peripheral vasodilation with marked hypotension and possibly reflex tachycardia. Management involves symptomatic treatment, support of cardiovascular function, and gastric lavage or activated charcoal if ingestion is recent. Intravenous calcium gluconate may be beneficial.

Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Consult a doctor. Amlodipine is excreted in human milk; lactation is not recommended.
