Description
Overview of the medicine
Lola 3 gm Powder contains Levocarnitine, an amino acid derivative involved in fatty acid metabolism. It is used to treat primary and secondary carnitine deficiencies, particularly in conditions like metabolic disorders or end-stage renal disease.
Uses & Indications
Dosage
Adults
For carnitine deficiency, typically 1-3 gm daily, divided into 2-3 doses. Dissolve one sachet (3 gm) in water or beverage and take orally.
Elderly
No specific dose adjustment required, follow adult dosage unless severe renal impairment is present.
Renal_impairment
In ESRD patients on hemodialysis, 1-3 gm daily post-dialysis, as advised by physician. Dosage may need adjustment in non-dialysis severe renal impairment.
How to Take
Dissolve the entire content of one sachet in a glass of water or other liquid (e.g., juice) and drink immediately. It can be taken with or without food.
Mechanism of Action
Levocarnitine facilitates the transport of long-chain fatty acids into the mitochondrial matrix, where they undergo beta-oxidation for energy production. It also helps remove toxic short- and medium-chain fatty acids that accumulate in metabolic disorders.
Pharmacokinetics
Onset
Therapeutic effects are not acutely defined but carnitine levels increase within hours of administration.
Excretion
Excreted primarily by the kidneys, mostly as unchanged levocarnitine and its acyl esters. Renal excretion is the major elimination pathway.
Half life
Approximately 17 hours.
Absorption
Rapidly absorbed orally, with a bioavailability of about 15-18%. Peak plasma concentrations are reached within 3-5 hours.
Metabolism
Minimal metabolism in humans; primarily converted to acylcarnitines and other esters by gut bacteria.
Side Effects
Contraindications
- Hypersensitivity to levocarnitine or any component of the formulation.
Drug Interactions
Warfarin
Concurrent use may potentiate the anticoagulant effect of warfarin, requiring close monitoring of INR.
Thyroid hormones
Levocarnitine may increase the activity of thyroid hormones; caution is advised in patients on thyroid replacement therapy.
Storage
Store in a cool (below 30°C) and dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Overdose symptoms may include gastrointestinal disturbances like nausea, vomiting, and diarrhea. Treatment is symptomatic and supportive. Levocarnitine is dialyzable.
Pregnancy & Lactation
Use during pregnancy only if clearly needed and the potential benefits outweigh the potential risks to the fetus. Levocarnitine is excreted in breast milk; use with caution during lactation and consult a physician.

Doctor Notes
- Confirm diagnosis of carnitine deficiency through appropriate biochemical tests before initiating levocarnitine therapy.
- Monitor patient's clinical response, plasma carnitine levels, and renal function regularly, especially in ESRD patients.
- Advise patients about the potential for body odor and gastrointestinal upset, which are typically dose-related.
- Adjust dosage based on patient's clinical and biochemical response.
Patient Guidelines
- Take this medicine exactly as prescribed by your doctor.
- Dissolve the powder completely in water or other liquid before drinking.
- Do not exceed the recommended dose.
- Inform your doctor about all other medications you are taking.
