Lopres 100 mg Tablet contains Metoprolol Tartrate, a beta-blocker used to treat high blood pressure, angina, and heart failure. It works by blocking certain natural chemicals in your body, like adrenaline, which affects the heart and blood vessels.
Uses & Indications
Dosage
Adults
Hypertension: Initially 50-100 mg/day in 1-2 divided doses, maintenance 100-400 mg/day. Angina: 50-100 mg 2-3 times daily. Myocardial Infarction: 100 mg twice daily. Heart Failure: Initially 25 mg once daily, titrate up to 200 mg/day.
Elderly
Start with lower doses and titrate slowly due to potential for increased sensitivity.
Renal_impairment
No dosage adjustment needed for mild-to-moderate renal impairment. Use with caution in severe impairment.
How to Take
Take orally, preferably with or immediately after food. Swallow the tablet whole with water. Do not crush or chew.
Mechanism of Action
Metoprolol is a cardioselective beta-1 adrenergic blocking agent. It selectively blocks beta-1 adrenergic receptors, primarily located in the heart, reducing heart rate and cardiac output, thereby lowering blood pressure. It also reduces renin release from the kidneys.

Primarily renal excretion (5% unchanged drug, remainder as metabolites).

3-7 hours (dose-dependent).
Absorption
Rapid and complete absorption (90%) from the GI tract. Bioavailability is 40-50% due to significant first-pass metabolism.
Metabolism
Primarily hepatic, extensive first-pass metabolism via CYP2D6.
Side Effects
Contraindications
- •Second or third-degree heart block
- •Sick sinus syndrome (unless permanent pacemaker is in place)
- •Cardiogenic shock
- •Decompensated heart failure
- •Severe bradycardia
- •Severe peripheral arterial disease
- •Hypotension
Drug Interactions
Clonidine
May potentiate rebound hypertension upon clonidine withdrawal.
MAO Inhibitors
May enhance the hypotensive effect and cause severe bradycardia.
Other Antihypertensives
Additive hypotensive effects.
NSAIDs (e.g., Ibuprofen, Indomethacin)
May reduce the hypotensive effect of metoprolol.
CYP2D6 inhibitors (e.g., Quinidine, Fluoxetine)
May increase metoprolol plasma concentrations.
Calcium Channel Blockers (e.g., Verapamil, Diltiazem)
Increased risk of bradycardia, hypotension, and AV block.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include severe bradycardia, hypotension, cardiac insufficiency, bronchospasm, and hypoglycemia. Management includes gastric lavage, activated charcoal, atropine for bradycardia, glucagon, IV fluids, vasopressors, and bronchodilators.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs risk. Metoprolol is excreted in breast milk; use with caution in nursing mothers, monitor infant for bradycardia and hypoglycemia.
