Description
Overview of the medicine
Desloratadine is a non-drowsy antihistamine that provides relief from allergy symptoms such as sneezing, runny nose, itchy eyes, and hives. It is used to treat seasonal and perennial allergic rhinitis and chronic idiopathic urticaria.
Uses & Indications
Dosage
Adults
One 5 mg tablet once daily.
Elderly
No dosage adjustment typically required, but caution is advised in elderly patients with renal or hepatic impairment.
Renal_impairment
For severe renal impairment (creatinine clearance < 30 mL/min), a reduced frequency (e.g., 5 mg every other day) is recommended.
How to Take
Take orally, with or without food, at any time of day.
Mechanism of Action
Desloratadine is a long-acting, selective peripheral H1-receptor antagonist. It inhibits histamine release from mast cells and reduces inflammatory mediators, thereby alleviating allergic symptoms.
Pharmacokinetics
Onset
Antihistaminic effects generally begin within 1-2 hours.
Excretion
Excreted in urine (approx. 40%) and feces (approx. 13%) as glucuronide conjugates and unchanged drug.
Half life
The mean plasma elimination half-life is approximately 27 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are reached in approximately 3 hours.
Metabolism
Extensively metabolized to 3-hydroxydesloratadine, an active metabolite, by CYP2D6 (though not a major pathway).
Side Effects
Contraindications
- Hypersensitivity to desloratadine, loratadine, or any component of the formulation.
- Severe renal or hepatic impairment (use with caution).
Drug Interactions
Cimetidine
May increase plasma concentrations of desloratadine, though usually not clinically significant.
Fluoxetine
May increase plasma concentrations of desloratadine, though usually not clinically significant.
Azithromycin
May increase plasma concentrations of desloratadine, though usually not clinically significant.
Erythromycin
May increase plasma concentrations of desloratadine, though usually not clinically significant.
Ketoconazole
May increase plasma concentrations of desloratadine, though usually not clinically significant.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment is recommended. Hemodialysis does not remove desloratadine.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit outweighs the potential risk to the fetus. Desloratadine is excreted into breast milk, so caution is advised during lactation.
