Overview of the medicine
Loratadine is a non-drowsy antihistamine used to relieve symptoms of seasonal allergies and chronic urticaria.
Uses & Indications
Dosage
Adults
10 mg (one tablet) once daily.
Elderly
No dosage adjustment is generally required for elderly patients with normal renal and hepatic function.
Renal_impairment
For patients with severe renal impairment (creatinine clearance < 30 mL/min) or hepatic impairment, a starting dose of 10 mg every other day is recommended.
How to Take
Take orally, with or without food. Do not chew or crush the tablet; swallow whole with water.
Mechanism of Action
Loratadine selectively blocks peripheral H1-histamine receptors, thereby inhibiting the effects of histamine and alleviating allergic symptoms without causing significant central nervous system effects.

Onset
Onset of action typically occurs within 1-3 hours.
Excretion
Approximately 40% excreted in urine and 42% in feces over 10 days, primarily as conjugated metabolites.
Half life
Loratadine: 8.4 hours (range 3-20 hours); Desloratadine (active metabolite): 27 hours (range 8.8-92 hours).
Absorption
Rapidly absorbed from the gastrointestinal tract. Food may delay absorption slightly.
Metabolism
Extensively metabolized in the liver by cytochrome P450 enzymes (CYP3A4 and CYP2D6) to its primary active metabolite, descarboethoxyloratadine (desloratadine).

Contraindications
- Hypersensitivity to loratadine or any component of the formulation.
- Hypersensitivity to desloratadine.
Drug Interactions
Cimetidine
Co-administration with cimetidine may increase plasma concentrations of loratadine and desloratadine, usually without clinical significance.
Erythromycin
Co-administration with erythromycin may increase plasma concentrations of loratadine and desloratadine, usually without clinical significance.
Ketoconazole
Co-administration with ketoconazole may increase plasma concentrations of loratadine and its active metabolite desloratadine, but this is generally not clinically significant.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include somnolence, tachycardia, and headache. In case of overdose, symptomatic and supportive treatment should be initiated immediately. Gastric lavage may be considered. Loratadine is not effectively removed by hemodialysis.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Excreted in breast milk, therefore not recommended during lactation. Consult a doctor before use.
