Description
Overview of the medicine
Lornoxicam is a non-steroidal anti-inflammatory drug (NSAID) of the oxicam class with analgesic and anti-inflammatory properties. It is used for symptomatic relief of pain and inflammation in various conditions.
Uses & Indications
Dosage
Adults
For osteoarthritis and rheumatoid arthritis: 8 mg once or twice daily, or 4 mg two or three times daily. Maximum recommended daily dose is 16 mg. For acute pain: Initial dose of 8-16 mg, followed by 8 mg 1-2 times daily.
Elderly
No specific dose adjustment is required, but caution should be exercised due to increased risk of adverse effects.
Renal_impairment
Max 8 mg/day. Use with caution in mild to moderate renal impairment. Contraindicated in severe renal impairment.
How to Take
Loricam tablets should be swallowed with a sufficient amount of liquid, preferably before meals. For patients with gastrointestinal discomfort, it can be taken during meals.
Mechanism of Action
Lornoxicam inhibits prostaglandin synthesis by inhibiting cyclooxygenase (COX-1 and COX-2) enzymes. This reduces the formation of inflammatory mediators, thereby exerting its analgesic, anti-inflammatory, and antipyretic effects.
Pharmacokinetics
Onset
Analgesic effect usually within 30 minutes.
Excretion
Approximately 2/3 is eliminated via the liver/bile and 1/3 via the kidneys, mainly as inactive metabolites.
Half life
Approximately 3-5 hours.
Absorption
Rapid and almost complete oral absorption; peak plasma concentrations achieved in 1-2 hours.
Metabolism
Extensively metabolized in the liver, primarily by hydroxylation via CYP2C9; metabolites are inactive.
Side Effects
Contraindications
- Hypersensitivity to lornoxicam or to any of the excipients, or to other NSAIDs (e.g., aspirin-induced asthma).
- Active or recurrent peptic ulcer/hemorrhage.
- Severe hepatic or renal impairment.
- Severe heart failure.
- Third trimester of pregnancy.

Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include nausea, vomiting, abdominal pain, drowsiness, and, rarely, gastrointestinal bleeding, coma, and acute renal failure. Management is symptomatic and supportive. Gastric lavage and activated charcoal may be considered.
Pregnancy & Lactation
Contraindicated in the third trimester of pregnancy due to potential for premature closure of the ductus arteriosus and renal dysfunction in the fetus. Not recommended during breastfeeding. Use in the first and second trimesters only if the potential benefit outweighs the risk.
