Description
Overview of the medicine
Losapot Plus 100 mg tablet is a combination medicine used to treat high blood pressure (hypertension). It contains Losartan, an angiotensin II receptor blocker, and Hydrochlorothiazide, a thiazide diuretic, which work together to lower blood pressure effectively.
Uses & Indications
Dosage
Adults
Usually one tablet once daily. May be increased if necessary after consultation with a physician.
Elderly
No initial dosage adjustment required for most elderly patients, but monitoring is advised.
Renal_impairment
Use with caution; not recommended for patients with severe renal impairment (creatinine clearance <30 mL/min).
How to Take
To be taken orally, with or without food, usually once daily. It is advisable to take it at the same time each day, preferably in the morning.
Mechanism of Action
Losartan blocks the vasoconstrictor and aldosterone-secreting effects of angiotensin II by selectively blocking the binding of angiotensin II to the AT1 receptor. Hydrochlorothiazide increases the excretion of sodium and chloride in the urine by inhibiting sodium reabsorption in the distal convoluted tubule, leading to diuresis and a reduction in blood volume.
Pharmacokinetics
Onset
Losartan: 1 hour, peak effect 6 hours. Hydrochlorothiazide: 2 hours, peak effect 4-6 hours. Combined effect typically within 3-6 hours.
Excretion
Losartan and its active metabolite are primarily excreted in the urine (35%) and feces (60%). Hydrochlorothiazide is primarily excreted unchanged in the urine (95%).
Half life
Losartan: 2 hours (active metabolite 6-9 hours). Hydrochlorothiazide: 5.8-14.8 hours.
Absorption
Losartan is well absorbed orally, with an average bioavailability of about 33%. Hydrochlorothiazide is also well absorbed orally (60-80%).
Metabolism
Losartan undergoes extensive first-pass metabolism by cytochrome P450 enzymes (CYP2C9 and CYP3A4) to an active carboxylic acid metabolite (E-3174). Hydrochlorothiazide is not metabolized.

- Hypersensitivity to Losartan, Hydrochlorothiazide, or sulfonamide-derived drugs
- Anuria
- Severe renal impairment
- Severe hepatic impairment
- Pregnancy (second and third trimesters)
- Concomitant use with Aliskiren in patients with diabetes or renal impairment
Drug Interactions
NSAIDs
May reduce the antihypertensive effect and increase the risk of renal impairment.
Lithium
Increased serum lithium concentrations and toxicity.
ACE Inhibitors/Aliskiren
Increased risk of hypotension, hyperkalemia, and renal impairment.
Alcohol, barbiturates, or narcotics
Potentiation of orthostatic hypotension.
Potassium-sparing diuretics/Potassium supplements
Increased risk of hyperkalemia.

Symptoms may include hypotension, tachycardia, bradycardia, electrolyte disturbances, and dehydration. Treatment is symptomatic and supportive, including intravenous fluids and electrolyte correction.
Pregnancy & Lactation
Contraindicated in the second and third trimesters of pregnancy due to fetal toxicity. Not recommended during lactation as both components are excreted in human milk.
