Description
Overview of the medicine
Losartan is an angiotensin II receptor blocker (ARB) used to treat high blood pressure (hypertension) and to protect the kidneys from damage due to diabetes. It also reduces the risk of stroke in patients with hypertension and an enlarged heart.
Uses & Indications
Dosage
Adults
Typically 50 mg once daily. Dosage can range from 25 mg to 100 mg daily, given as a single dose or in two divided doses.
Elderly
No initial dosage adjustment is usually necessary. Consider a lower starting dose (e.g., 25 mg once daily) for very elderly or frail patients.
Renal_impairment
No initial dose adjustment is required for patients with mild to moderate renal impairment. For severe impairment, monitor blood pressure and kidney function carefully.
How to Take
Take orally, with or without food. It can be taken at any time of day, but should be taken at the same time each day.
Mechanism of Action
Losartan selectively blocks the binding of angiotensin II to the AT1 receptor found in various tissues, including vascular smooth muscle and the adrenal gland. This blockage inhibits the vasoconstrictive and aldosterone-secreting effects of angiotensin II, leading to vasodilation and reduced blood pressure.
Pharmacokinetics
Onset
Antihypertensive effect begins within approximately 1 hour, with peak effect achieved in 6 hours. Maximal blood pressure reduction may take 3-6 weeks.
Excretion
Approximately 35% excreted in the urine and 60% in the feces as unchanged drug and metabolites.
Half life
Losartan: approximately 2 hours. Active metabolite (E-3174): approximately 6-9 hours.
Absorption
Well absorbed from the gastrointestinal tract. Bioavailability is approximately 33%.
Metabolism
Undergoes extensive first-pass metabolism by cytochrome P450 enzymes (CYP2C9 and CYP3A4) in the liver to form an active carboxylic acid metabolite (E-3174), which is more potent than losartan.
Side Effects
Contraindications
- Hypersensitivity to losartan or any component of the formulation
- Pregnancy (especially 2nd and 3rd trimesters)
- Concomitant use with aliskiren in patients with diabetes
Drug Interactions
NSAIDs
May reduce the antihypertensive effect of losartan and increase the risk of renal impairment.
Lithium
Increased serum lithium concentrations and toxicity have been reported. Monitor lithium levels.
Potassium-sparing diuretics
May lead to hyperkalemia (high potassium levels).
Storage
Store at controlled room temperature (20-25°C), away from moisture and direct light. Keep out of reach of children.
Overdose
Symptoms may include hypotension (low blood pressure) and tachycardia (rapid heartbeat); bradycardia (slow heartbeat) could occur. Treatment is symptomatic and supportive. If ingestion is recent, induce emesis or perform gastric lavage. Hemodialysis is unlikely to be effective.
Pregnancy & Lactation
Pregnancy Category D (2nd and 3rd trimesters). Can cause harm to the fetus. Discontinue immediately if pregnancy is detected. Not recommended during breastfeeding, as it may pass into breast milk.
