Description
Overview of the medicine
Losart Plus 100 mg tablet is a combination medicine containing Losartan (an angiotensin II receptor blocker) and Hydrochlorothiazide (a thiazide diuretic). It is primarily used to treat high blood pressure (hypertension) and to reduce the risk of stroke in patients with high blood pressure and an enlarged heart (left ventricular hypertrophy).

Dosage
Adults
The usual starting dose is one Losart Plus 50/12.5 mg tablet once daily. For the 100/25 mg strength, it's typically one tablet (Losartan 100 mg / Hydrochlorothiazide 25 mg) once daily, adjusted based on blood pressure response.
Elderly
No specific initial dosage adjustment is typically needed for elderly patients, but careful monitoring of renal function and electrolytes is recommended.
Renal_impairment
Use with caution in patients with moderate renal impairment (creatinine clearance 30-50 mL/min). Contraindicated in patients with severe renal impairment (creatinine clearance <30 mL/min) or anuria.
How to Take
Take orally, with or without food. It is usually taken once daily, preferably in the morning.
Mechanism of Action
Losartan works by blocking the action of angiotensin II, a natural substance that constricts blood vessels, leading to relaxed blood vessels and lower blood pressure. Hydrochlorothiazide is a diuretic that helps the body get rid of extra salt and water, which also helps to lower blood pressure.

Onset
Losartan: Peak effect within 6 hours. Hydrochlorothiazide: Onset of diuresis within 2 hours.
Excretion
Losartan and its active metabolite are eliminated via urine and feces. Hydrochlorothiazide is primarily excreted unchanged in the urine.
Half life
Losartan: ~2 hours (parent drug), ~6-9 hours (active metabolite E-3174). Hydrochlorothiazide: ~5.6-14.8 hours.
Absorption
Losartan is well absorbed orally, with approximately 33% systemic bioavailability. Hydrochlorothiazide is absorbed orally, with approximately 65-75% bioavailability.
Metabolism
Losartan is extensively metabolized in the liver, primarily by CYP2C9 and CYP3A4, to an active carboxylic acid metabolite (E-3174). Hydrochlorothiazide is not metabolized.
Side Effects
Contraindications
- •Hypersensitivity to Losartan, Hydrochlorothiazide, or any sulfonamide-derived drugs.
- •Anuria (inability to produce urine).
- •Severe renal impairment (creatinine clearance <30 mL/min).
- •Severe hepatic impairment.
- •Pregnancy (especially second and third trimesters).
Drug Interactions
Lithium
May increase serum lithium levels, leading to toxicity.
Alcohol, barbiturates, or narcotics
May potentiate orthostatic hypotension (sudden drop in blood pressure upon standing).
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the antihypertensive effect and increase the risk of renal impairment.
Potassium-sparing diuretics/potassium supplements
May lead to increased serum potassium levels.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypotension (low blood pressure), tachycardia (fast heart rate), bradycardia (slow heart rate), and electrolyte disturbances (e.g., hypokalemia, hyponatremia) and dehydration due to the diuretic component. Treatment is supportive and symptomatic.
Pregnancy & Lactation
Losart Plus is contraindicated in the second and third trimesters of pregnancy due to the risk of fetal injury and death. It is not recommended during breastfeeding as both Losartan and Hydrochlorothiazide are excreted in breast milk.
