Overview of the medicine
Losartan 50 mg Tablet is an angiotensin II receptor blocker (ARB) used to treat high blood pressure (hypertension), reduce the risk of stroke in patients with hypertension and left ventricular hypertrophy, and treat kidney disease in patients with type 2 diabetes and a history of hypertension.
Uses & Indications
Dosage
Adults
Initial dose is usually 50 mg once daily. May be increased to 100 mg once daily if needed.
Elderly
No initial dosage adjustment is typically needed for most elderly patients. Consider starting with 25 mg once daily if volume depletion is suspected.
Renal_impairment
No initial dosage adjustment required for mild to moderate renal impairment. For severe renal impairment or dialysis, a lower starting dose (e.g., 25 mg once daily) may be considered.
How to Take
Orally, with or without food, usually once daily. It is recommended to take it at the same time each day.
Mechanism of Action
Losartan selectively blocks the binding of angiotensin II to the AT1 receptor in various tissues, preventing vasoconstriction, aldosterone secretion, and other effects of angiotensin II, thereby lowering blood pressure.

Onset
Antihypertensive effect begins within 6 hours, maximal effect within 3-6 weeks.
Excretion
Approximately 35% excreted in urine and 60% in feces, primarily as unchanged drug and active metabolite.
Half life
Losartan: 2 hours; active metabolite (E-3174): 6-9 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability is approximately 33%.
Metabolism
Undergoes extensive first-pass metabolism by cytochrome P450 enzymes (CYP2C9 and CYP3A4) to an active carboxylic acid metabolite (E-3174) and inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to losartan or any component of the formulation
- •Pregnancy (especially second and third trimesters)
- •Concomitant use with aliskiren in patients with diabetes or renal impairment
Drug Interactions
NSAIDs
May reduce antihypertensive effect and increase risk of renal impairment.
Lithium
May increase serum lithium levels and toxicity.
Aliskiren
Contraindicated in diabetic or renal impaired patients due to increased risk of adverse events.
Potassium-sparing diuretics
May increase risk of hyperkalemia.
Storage
Store at room temperature (below 30°C), protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include hypotension (low blood pressure) and tachycardia (rapid heartbeat); bradycardia (slow heartbeat) may occur. Treatment is symptomatic and supportive, including intravenous fluids and vasopressors if severe hypotension occurs.
Pregnancy & Lactation
Pregnancy: Category D. Contraindicated during the second and third trimesters due to risk of fetal injury or death. Lactation: Not recommended as it is unknown if losartan is excreted in human milk.
