Description
Overview of the medicine
Losek (Omeprazole) is a proton pump inhibitor used to treat conditions caused by excess stomach acid, such as heartburn, acid reflux, and ulcers.
Uses & Indications
Dosage
Adults
For GERD: 20 mg once daily for 4-8 weeks. For Duodenal Ulcer: 20 mg once daily for 2-4 weeks. For Gastric Ulcer: 20 mg once daily for 4-8 weeks. For Zollinger-Ellison Syndrome: Initial 60 mg once daily, adjusted as needed.
Elderly
No dose adjustment generally required, but caution in impaired hepatic function.
Renal_impairment
No dose adjustment necessary.
How to Take
Usually taken once daily in the morning before a meal. Swallow capsules whole; do not chew or crush. For patients unable to swallow, capsules can be opened and granules mixed with a small amount of liquid or soft food.
Mechanism of Action
Omeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase (proton pump) in gastric parietal cells, thereby reducing gastric acid secretion.

Excretion
Mainly excreted in urine (approx. 80%) and feces (approx. 20%) as metabolites.
Half life
0.5-1 hour (plasma elimination), but duration of acid suppression is longer due to irreversible binding.
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 30-40% at single doses, increasing to about 60% with repeated doses.
Metabolism
Extensively metabolized in the liver by cytochrome P450 enzymes, primarily CYP2C19 and CYP3A4.

Contraindications
- Hypersensitivity to omeprazole or other benzimidazoles (e.g., esomeprazole, lansoprazole)
- Concomitant use with nelfinavir

Increased digoxin bioavailability.
Clopidogrel
Reduced antiplatelet effect (CYP2C19 inhibition).
Methotrexate
Increased methotrexate levels.
Nelfinavir, Atazanavir
Decreased plasma concentrations of these antivirals.
Warfarin, Diazepam, Phenytoin
Increased plasma levels due to inhibition of CYP2C19.
