Description
Overview of the medicine
Lotenate 0.5% Suspension is an ophthalmic corticosteroid used to treat inflammation and pain associated with eye surgery or allergic conjunctivitis. It reduces swelling, redness, and itching.
Uses & Indications
Dosage
Adults
One to two drops instilled into the conjunctival sac of the affected eye(s) four times daily, beginning 24 hours after surgery and continuing throughout the first 2 weeks of the post-operative period. For allergic conjunctivitis, usually one drop four times daily.
Elderly
No specific dosage adjustment required for elderly patients.
Renal_impairment
No specific dosage adjustment required as systemic absorption is minimal.
How to Take
For ocular use only. Shake well before use. Do not touch the tip of the dropper to any surface to avoid contamination. Apply drops as directed into the conjunctival sac.
Mechanism of Action
Loteprednol Etabonate is a 'soft' corticosteroid. It works by inhibiting the release of substances in the body that cause inflammation. Unlike traditional corticosteroids, it is rapidly metabolized to inactive metabolites, reducing the risk of systemic side effects.
Pharmacokinetics
Onset
Within hours of administration, peak effect often within days.
Excretion
Primarily excreted via metabolism products; minimal intact drug excreted.
Half life
Plasma half-life is short, rapidly metabolized.
Absorption
Minimal systemic absorption after ocular administration. Primarily absorbed locally in ocular tissues.
Metabolism
Extensively metabolized to inactive carboxylic acid metabolites by esterases in ocular tissues and systemic circulation.
Side Effects
Contraindications
- Viral diseases of the cornea and conjunctiva (e.g., herpes simplex keratitis)
- Mycobacterial infection of the eye
- Fungal infections of ocular structures
- Hypersensitivity to the active substance or to any of the excipients
Drug Interactions
Other topical corticosteroids
Concurrent use may increase intraocular pressure.
Immunosuppressants (e.g., Cyclosporine)
Possible increased risk of systemic side effects if significant systemic absorption occurs (unlikely with ophthalmic use).
Storage
Store at room temperature (15°C to 30°C). Do not freeze. Keep away from direct light and moisture. Keep out of reach of children.
Overdose
An acute overdose is unlikely to cause acute problems due to the route of administration and minimal systemic absorption. If accidentally ingested, symptomatic and supportive treatment should be initiated.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Caution should be exercised when administered to a nursing mother. Consult a doctor.

Doctor Notes
- Educate patients on proper instillation technique and the importance of not discontinuing abruptly.
- Monitor IOP if treatment extends beyond 10 days.
- Consider the risk/benefit ratio, especially in patients with glaucoma or a history of herpes simplex keratitis.
