Description
Overview of the medicine
Lotenate Plus is an ophthalmic suspension combining a corticosteroid (Loteprednol Etabonate) and an aminoglycoside antibiotic (Tobramycin). It is used to treat steroid-responsive inflammatory ocular conditions where bacterial ocular infection or a risk of bacterial ocular infection exists.
Uses & Indications
Dosage
Adults
Instill 1 drop into the affected eye(s) 3-4 times daily. In severe cases, dosage may be increased to 1 drop every hour for the first 24-48 hours, then tapered as inflammation subsides.
Elderly
No specific dosage adjustment is necessary for elderly patients.
Renal_impairment
No dosage adjustment is necessary for ophthalmic administration as systemic absorption is minimal.
How to Take
Shake the bottle well before each use. Tilt your head back, pull down your lower eyelid to form a pocket, and instill one drop into the pocket. Do not touch the dropper tip to any surface (eye, finger, etc.) to avoid contamination. Close your eye gently and apply pressure to the tear duct for a minute to prevent drainage. If using other eye drops, wait at least 5 minutes between applications.
Mechanism of Action
Loteprednol Etabonate is a corticosteroid that inhibits the inflammatory response by suppressing migration of fibroblasts and leukocytes, reversing capillary permeability, and lysosomal stabilization. Tobramycin is an aminoglycoside antibiotic that inhibits bacterial protein synthesis by irreversibly binding to the 30S ribosomal subunit, leading to bacterial cell death.

Onset
Anti-inflammatory and antibacterial effects can be observed within hours of administration.
Excretion
Systemically absorbed components (if any) are primarily excreted unchanged in urine, but ocular route leads to minimal systemic exposure.
Half life
Systemic half-life not clinically relevant for ophthalmic use due to minimal absorption.
Absorption
Minimal systemic absorption after ocular administration. Loteprednol Etabonate is rapidly metabolized to inactive compounds. Tobramycin also has limited systemic absorption from the eye.
Metabolism
Loteprednol Etabonate undergoes extensive metabolism to inactive carboxylic acid metabolites. Tobramycin is not significantly metabolized.

Ocular overdose is unlikely to cause acute problems. If accidentally ingested, drink fluids to dilute. If an ocular overdose occurs, the eye can be flushed with lukewarm water.
