Description
Overview of the medicine
Lotenib 100 mg Capsule is a targeted antineoplastic agent used in the treatment of various cancers by inhibiting specific tyrosine kinases crucial for cancer cell growth and survival.
Uses & Indications
Dosage
Adults
100 mg once daily, taken orally with or without food. Adjust dose based on tolerability and clinical response.
Elderly
No specific dose adjustment required for elderly patients, but monitor for adverse reactions.
Renal_impairment
No dose adjustment needed for mild to moderate renal impairment. Use with caution in severe impairment.
How to Take
Take the capsule orally, at approximately the same time each day. Can be taken with or without food. Swallow whole; do not open, crush, or chew.
Mechanism of Action
Lotenib selectively inhibits multiple receptor tyrosine kinases (RTKs) and non-receptor tyrosine kinases (nRTKs) involved in cell proliferation, angiogenesis, and metastasis. By blocking these signaling pathways, Lotenib induces apoptosis and inhibits tumor growth.
Pharmacokinetics
Onset
Clinical effects usually observed within a few weeks of consistent dosing.
Excretion
Mainly excreted via feces (approx. 80%) with a smaller portion (approx. 10%) eliminated renally.
Half life
Approximately 18-24 hours, allowing for once-daily dosing.
Absorption
Rapidly absorbed with peak plasma concentrations reached within 2-4 hours. Bioavailability is approximately 60-70%.
Metabolism
Primarily metabolized in the liver by CYP3A4 enzymes.
Side Effects
Contraindications
- Hypersensitivity to Lotenib or any excipients.
- Severe hepatic impairment.
Drug Interactions
Warfarin
Increased risk of bleeding due to potential inhibition of CYP2C9.
CYP3A4 Inducers (e.g., rifampicin)
May decrease Lotenib plasma concentrations, potentially reducing efficacy.
CYP3A4 Inhibitors (e.g., ketoconazole)
May increase Lotenib plasma concentrations, requiring dose reduction.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
In case of overdose, seek immediate medical attention. Management includes symptomatic and supportive care. There is no specific antidote.
Pregnancy & Lactation
Category D in pregnancy (evidence of fetal risk). Should not be used during pregnancy unless the potential benefit justifies the potential risk to the fetus. It is not known whether Lotenib is excreted in human milk; avoid breastfeeding during treatment and for a period after the last dose.
