Description
Overview of the medicine
Lotepred-Plus 0.5/0.3 Suspension is an ophthalmic combination medication containing Loteprednol Etabonate (a corticosteroid) and Gatifloxacin (a fluoroquinolone antibiotic). It is used to treat inflammatory ocular conditions where there is a risk of bacterial infection or an existing bacterial infection.
Uses & Indications
Dosage
Adults
Instill 1-2 drops into the conjunctival sac of the affected eye(s) 2-4 times daily, or as directed by a physician. The dosage may be tapered down gradually based on clinical response.
Elderly
No specific dosage adjustment is required for elderly patients.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment due to minimal systemic absorption.
How to Take
For topical ophthalmic use only. Shake the bottle well before each use. Tilt your head back, pull down the lower eyelid to form a pocket, and instill the prescribed number of drops. Do not touch the dropper tip to your eye or any other surface to avoid contamination.
Mechanism of Action
Loteprednol Etabonate works as a corticosteroid, inhibiting the release of inflammatory mediators (e.g., prostaglandins, leukotrienes), thereby reducing inflammation, swelling, and pain. Gatifloxacin is a broad-spectrum fluoroquinolone antibiotic that inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes for bacterial DNA replication, transcription, repair, and recombination, leading to bacterial cell death.
Pharmacokinetics
Onset
Anti-inflammatory and antibacterial effects can be observed within minutes to hours after administration.
Excretion
Metabolites of Loteprednol are primarily excreted via urine. Unabsorbed Gatifloxacin is excreted unchanged, primarily through the kidneys.
Half life
Loteprednol Etabonate is rapidly metabolized to inactive compounds. Gatifloxacin's ocular half-life is not precisely defined but sustains therapeutic concentrations locally.
Absorption
Systemic absorption of both Loteprednol Etabonate and Gatifloxacin from ocular administration is minimal. Loteprednol is designed for localized action with rapid metabolism to inactive forms.
Metabolism
Loteprednol Etabonate undergoes rapid metabolism via ester hydrolysis to inactive carboxylic acid metabolites. Gatifloxacin is minimally metabolized.
Side Effects
Contraindications
- Hypersensitivity to any component of the medication or to other quinolones or corticosteroids.
- Acute untreated purulent bacterial infections of the eye, viral diseases of the cornea and conjunctiva (including epithelial herpes simplex keratitis, vaccinia, varicella), mycobacterial infection of the eye, and fungal diseases of ocular structures.
Drug Interactions
Systemic interactions
Minimal systemic absorption, so clinically significant systemic drug interactions are unlikely.
Other ophthalmic preparations
If using other ophthalmic medications, wait at least 5-10 minutes between applications to prevent wash-out.
Storage
Store at room temperature (15-30°C). Protect from light. Do not freeze. Keep out of reach of children.
Overdose
Overdose is unlikely with topical ophthalmic administration. If accidentally ingested, supportive measures may be necessary. Contact a poison control center or seek medical attention if symptoms occur.
Pregnancy & Lactation
Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. Caution should be exercised when administered to a nursing mother. Consult your doctor before use.

- Intraocular pressure (IOP) monitoring during prolonged treatment (e.g., >10 days) with corticosteroids.
- Regular ophthalmologic examinations including slit lamp biomicroscopy.
Doctor Notes
- Emphasize the importance of shaking the bottle before use.
- Educate patients on potential risks of prolonged steroid use (e.g., IOP elevation, cataract formation).
- Advise against wearing contact lenses during treatment unless directed by the ophthalmologist.
Patient Guidelines
- Shake the bottle well before each use.
